l. NARRATIVE

SUMMARY

OF OPERATING EXPERIENCES l Limerick Unit 1 began the raonth of July at a nominal 7% of rated thermal power. On July 3 at 0330 hours0.00382 days <br />0.0917 hours <br />5.456349e-4 weeks <br />1.25565e-4 months <br />, the main turbine generator was synchronized to the grid. On June 5, the reactor achieved -

100% of rated thermal power.' On July 6, reactor power was briery reduced to 95% for a control j

rod patten adjustment. On July 9, reactor power was briefly reduced to 98% of rated thermal pnwer due to high turbine back pressure. - On July 12, reactor power was briefly reduced to 95% for a contrca rod pattern adjustment. On July 15, reador power was briefly reduced to 98% due to high turbine back pressure. On July 20 and 27, reactor power was briefly reduced to 95% for a control ,

rod pattom adjustment. Limerick Unit 1 ended the month of July at a nominal 100% of rated thermal power.

Operational events that c arred during the month of July included:

- On July 5, during the performarx:e of the Standby Gas Treatment System (SGTS) ,

flow test the Reactor Water Cleanup (RWCU) System isolated. The isolation was '

due to a RWCU regenerative heat exchanger room high temperature signal used-for steam leak detection. Thc high temperature signal was created by several small steam leaks in the room in conjunction with the ventilation isolation necessary to perform the SGTS flow test. A four-(4) hour notification was made for an Engineered Safety Feature (ESF) actuation.

I. NARRATIVE

SUMMARY

OF OPERATING EXPERIENCES

~ Limerick Unit 2 began the month of July in OPCON 4. The unit had been shut down during the '

month of June for a routine maintenance outage, On July 10, the unit entered OPCON 2 at 0615 '3

hours and criticality was achieved at 1810 hours0.0209 days <br />0.503 hours <br />0.00299 weeks <br />6.88705e-4 months <br />. 5 July 12 at 1119 hours0.013 days <br />0.311 hours <br />0.00185 weeks <br />4.257795e-4 months <br />, the main turbine generator was synchronized to the grid. On July 15.  ;21 hours2.430556e-4 days <br />0.00583 hours <br />3.472222e-5 weeks <br />7.9905e-6 months <br /> main turbine condenser vacuum began to rapidly degrade, leading to a turbine trip are actor automatic scram at 0931 hours0.0108 days <br />0.259 hours <br />0.00154 weeks <br />3.542455e-4 months <br />. The 1 unit was subsequently placed in cold shutdown for Investigation and repair. On July 20, the drain line, which caused the loss of vacuum, was repaired snd other maintenance continued. On July 23, the unit entered OPCON 2 at 0338 hours0.00391 days <br />0.0939 hours <br />5.588624e-4 weeks <br />1.28609e-4 months <br /> and achieved criticality at 0615 hours0.00712 days <br />0.171 hours <br />0.00102 weeks <br />2.340075e-4 months <br />. On July 24 at f 1725 hours0.02 days <br />0.479 hours <br />0.00285 weeks <br />6.563625e-4 months <br />, the main turbine generator was synchronized to the grid. On July 27, the reactor achieved 100% of rated thermal power. On July 29 at 1247 hours0.0144 days <br />0.346 hours <br />0.00206 weeks <br />4.744835e-4 months <br />, reactor power was reduced to a nominal 60% due to a control rod drift problem identified.during testing. ' Power was further reduced to 50% to establish a new control rod pattern for ascension to full power. On July 31, the reactor achieved 100% of rated thermal power.

Operational events that occurred during the month of July included:

On July 13, during preparations to work on the '2C' Reactor Enclosure Exhaust Fan, (

the air supply line to a solenoid valve (SV 076-255C) that supplies air to the exhaust fan blade positioner was sheared off. The loss of air caused the '2A' and '2B' Reactor Enclosure Exhaust Fans to cycle to minimum blade pitch,' causing a positive reactor enclosure differential pressure. The excessive positive pressure i caused the failure of a reactor enclosure blowout panel, which resulted in a loss of.

secondary containment. Maintenance repaired the blowout panet and secondary containment was restored -within the allowable. Technical Specification Action Statement timo limits.

On July 15, the Main Turbine tripped on low main condenser vacuum causing an automatic reactor shutdown. The loss of condenser vacuum was caused by a failed Main Turbine lube oil drain line which penetrates the condenser shell. The Main Condenser was blocked and the lube oil drain line was repaired.~ Supports were added on all the lube oil drain lines in the condenser to prevent future failures.

On July 29, with the reactor at 100% power, control rod 30 31 began to drift out following a single notch withdrawal signal during the control rod exercise test. The Reactor Operator was able to give the control rod an Insert signal to fully insert the l control rod. The control rod was declared inoperable and isolated for Technical Specification compliance. The control rod has remained inoperable pending further I testing and operability verification.
