September 15, 1992, through September 14, 1993 1.

In accordance with Environmental Technical Specification (ETS) 5.3.C, the j

facility design and operational changes were reviewed for the potential affect on the environment. Review of facility design and operational changes from September 15, 1992, through September 14, 1993, that could have affected the environment was performed. Projects considered to have i

potential impact on'the environment included: (1) those involving excavation, construction, or asbestos removal; and (2) those resulting in new or increased discharges to outside drains. The-review identified and

documented a basis that the design and operational' changes did not involve an unreviewed environmental question. A copy of this study is attached.

The following information is provided as required by ETS 5.4.1.

All ETS noncompliances and the corrective actions taken to remedy them:

Changes made to applicable state and federal permits and certifications:

Tennessee Baseline General Permit for Storm Water Discharges Associated i

with Industrial Activity, TNR000015, was isnued on November 1, 1992.

This permit ensures monitoring of storm water discharges through discharge points that are not included in National Pollutant Discharge Elimination System (NPDES) Permit No. TN0026450.

ATTACHMENT TO ENCLOSURE STUDY OF SEQUOYAH NUCLEAR PLANT (SQN)

DESIGN AND OPERATIONAL CHANGES BETWFEN SEPTEMBER 15, 1992, AND SEPTEMBER 14, 1993 FOR EFFECTS ON THE ENVIRONMENT j

During this report period, the facility design and operational changes and major maintenance and modification projects have been reviewed for potential i

affect to the environment as described below. The criteria used to identify-those projects with a potential for environmental effects included:

(1) those involving excavation, construction, or asbestos removal; and (2) those resulting in new or increased discharges to.outside drains.

The following projects were reviewed and evaluated in accordance with the above criteria for potential environmental impact.

New Issues

'i Forty-five new project packages were reviewed during this report period. All of those projects were designed to comply with regulatory permitting processes and none of them involved any environmental impacts that ware not reviewed in the original environmental impact atatement.

b.

Special Tests

There were no special tests conducted during this period that met the i

environmental impact criteria.

c.

Temporary Alterations i

There were no temporary alteratione made during this period that met the environmental impact criteria.

Design and operational changes with a potential for causing new or j

-increased discharges to outside drains, including runoff from excavation, were reviewed and determined to be within the scope of the SQN National Pollutant Discharge Elimination System (NPDES) and Stormwater Permits.

Monthly discharge monitoring reports were submitted as required by the SQN l

NPDES permit.

Permit excursions were attributed to either.

- I natural causes, equipment malfunctions, or operational errors and determined to be within the purview of the NPDES permit and associated environmental evaluations.

e In summary, there have been no facility design or operational changes from September 15, 1992, to September-14, 1993, that have resulted in an unreviewed-environmental question.
