I Total Residual Chlorine Limit exceeded on 5/18/98.

EVENT DESCRIPTION On 5/18/98 at 17:25, during raw cooling water (RCW) chlorination inception, SQN's NPDES calculated mstantaneous maximum release concentration (0.058 ppm) was exceeded. Sodium hypochlorite was released at the NPDES permitted Outfall, DSN-101, at Tennessee River mile 483.65. Outfall 101 was sampled at 19:00,21:00,22:38 on 5/18/98 and at 00:30 on 5/19/98. All samples measured at DSN-101 werc < 0.1 ppm. The cause of the noncompliance is suspected to be " leak through" from an adjacent i
hypochlorite injection pump being used. The chronology of the event is as follows.
5/18/98 @ 15:23.
Raw cooling water chlorination began 5/18/98 @ 17:25.
A routine sample was collected by site chemistry. Analysis result for TRC was 1
l calculated to be < 0.093 ppm.122 minutes may have elapsed with TRC at this level.
5/18/93 @ 17:30.
A second sample was collected. Calculated TRC was < 0.087 ppm. TRC concentration was at this level for 5 minutes.
j 5/18/98 @ 18:14 Chemical injection was terminated.
5/18/98 @ 18:30 Raw cooling water was sampled for the third time. The calculated TRC was j
< 0.102 ppm. (60 minutes).
i 5/18/98 @ 19:30 The fourth RCW sample was collected and calculated to be 0.084 ppm (60 minutes).
5/18/98 @ 20:35 The fifth RCW sample was collected and calculated to be 0.078 ppm at 20:35 (65 minutes).
i 5/18/98 @ 2130 Calculated RCW TRC was < 0.002 ppm.
The maximum period of noncompliance could have been as much as 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and 7 minutes.
1 ACTIONS TAKEN Sodium hypochlorite injection was terminated at 18:14 on 5/18/98. Ifinjection of hypochlorite had stopped 18:14, there should have been no measurable residual by 19:30 due to system demand. SQN l
Operations personnel isolated the discharge valve on an adjacent hypochlorite injection pump at 20:45.
l By 21:50, calculated DSN-101 RCW TRC was < 0.002 ppm. It is believed that a check valve failed on the adjacent pump. The pump was not operating, but the failure of the valve may have allowed sodium hypochlorite to drain into the RCW system and be the source of high TRC reading past the point where they should have been well within limits. Total Residual Chlorine (TRC) was not detectable (< 0.1 ppm) on samples collected at DSN-101 at any time. There was no observable aquatic distress at any time. There are no known or anticipated acute or chronic risks associated with the emergency. No medical attention was necessary.

STEPS TAKEN TO PREVENT RECURRENCE OF THE DISCHARGE Actions were initiated to perform necessary repairs to the pump (P-9) which was in use for sodium hypochlorite delivery during the incident. These repairs have been completed.
l The inlet and outlet of the pump (P-5) suspected ofleaking through have been isolated.
1 SQN Operation's implementing procedure specified valve alignments will be verified as correct.
SQN Chemistsy procedure for RCW/RSWkficrobiologically Induced Corrosion / Mollusk Control will be revised to check the TRC concentration as it enters the system on an approximate hourly basis until a constant (i.e.120%) TRC concentration is established. This surveillance will be required on chlorination initiation and/or pump adjustment or venting. Investigation of the feasibility ofinstalling online instrumentation to monitor TRC is also underway.
l' Thorough pre-job briefings of Chemistry and Operations personnel will be conducted prior to RCW chlorination resumption. Operations personnel will understand that isolation of this system is to include valve closures as well as termination of pump operation.. Che mistry personnel are to briefed on obtaining hourly samples or " read-outs" until concentrations are constant (i.e. t 20%) on chlorination initiation and/or pump adjustment orventing.

Failure to monitor DSN 110 temperature on a daily basis (6/5/98 - 6/8/98) as required by National Pollutant Discharge Elimination System (NPDES) Permit TN0026450 EVENT DESCRIPTION On June 8,1998 at 03:00, Sequoyah was in Open Mode operation when site Operations noted that there was inadequate Essential Raw Cooling Water (ERCW) flowing into the Diffuser Pond to allow a radioactive release. Investigation revealed that the middle of three gates whose function controls flow from the Cooling Tower Basin into the Cold Water Return Channel and ultimately DSN-110 discharge.
This configuration allowed a portion of the water in the Cooling Tower Basin to discharge down the Cold Water Return Channel. There was no Cooling Tower Blowdown as Cooling Towers were not in service and Condenser Cooling Water was aligned to the Diffuser Pond with discharge through Outfall 101. 2.68 inches of rain had fallen within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> on 6/5/98. This caused the water level in the Cooling Tower Basin to be high. A new control panel for the gate had been installed. The gate control was wired backwards. Therefore, when the gate simcture was re-energized at about 04:00 on 6/5/98, the gate opened. Subsequently, ERCW chemical treatment with Towerbrom* occurred for a 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> period, lasting frorn about 10:00 until about 14:00. The maximum residual in the ERCW system was 0.2 mg/L in Train A and 0.3 mg/L in Train B. ERCW system TRC was < 0.1 mg/L when sampled at 17:15 and 17:18 on June 5* There was no chemical treatment on June 6* or June 7*
The NPDES Permit requires monitoring of Outfall 110 only during Closed Mode operation which is normally the only time that significant flow through Outfall 110 occurs. During Closed Mode operation, a temperature reading is required once per day and a TRC is required once per 7 days. Because it was not known that the gate was open from Friday, June 5* until Sunday, June 7*, no temperature reading or TRC measurement was taken. The amount of the discharge was less than 27,000 gpm which, compared to the normal discharge of over 1,000,000 gpm, is a very small amount and would add relatively little heat to the discharge. In the early morning hours of June 7*,1998, based on Control Room instrumentation, over 15,000 gpm continued to flow to the Diffuser Pond. By June 8*, at 0300, Control Room Instrumentation showed less 15,000 gpm flowing to the Diffuser Pond. Based on the rainfall amount, the relatively low L
TRC concentration in the ERCW system, the low flow and presence of organic material in the Cooling j
Tower Basin and Cold Water Return Channel, it is our opinion that TRC at Outfall 110 was below the NPDES Permit limit for Closed Mode Operation of 0.1 mg/L at all times.
PERIOD OF EVENT I
i Control Room instrumentation indicated that ERCW was flowing into the Diffuser Pond at an adequate
]
amount to allow a radioactive release after midnight and into the early morning of Sunday, June 7,1998.
This indicates that over 15,000 gpm continued to flow to the Diffuser Pond for normal discharge to Outfall 101. However, about 06:00 on June 7, Nuclear Security reported to Fire operations that there was
" smoke" in the vicinity of the Low Level Radwaste Storage Building. Fire Operations investigated, determined that the " smoke" was actually fog from the river due to cool weather conditions. Fire Operations saw, but did not report the flow through Outfall 110 at 06:30 on June 7* The gate was i
manually closed, isolating the source of flow to Outfall 110 on June 8* at 03:00. The event duration is believed to have been approximately 71 hours8.217593e-4 days <br />0.0197 hours <br />1.173942e-4 weeks <br />2.70155e-5 months <br />.

O STEPS TAKEN TO PREVENT RECURRENCE OF TIIE DISCIIARGE To prevent recurrence of such an event, Sequoyah will ensure the following:
1.
Verification that the entry gate to the Cold Water Return Channel is closed will be made by visually observing the gates and the local motor indication lights daily until procedures are approved to de-energize the gate motors.
2.
Procedures will be revised to maintain gate motors in a de-energized status unless Closed Mode Operation is necessary.
3.
Chemistry personnel will receive direction on verification of regulatory related information.
4.
Maintenance personnel will be counseled on verification of work performed,
