2. Evaluation of Licensee Action with Regard to IE Circulars

(Open) IE Circular 78-18: UL Fire Test. Licensee documents DSN 1109780038

and DSN 0108805086 were reviewed. Sargent and Lundy stated that this

circular would be evaluated when the final results of the fire test

were distributed as stated in the circular. However, a supplemental

circular was not issued. The inspector determined that the final test

results were incorporated in NUREG CR/0596 (SAND No. 78-2238). This

circular will remain open pending the licensee's review of NUREG CR/0596.

(Closed) IE Circular 79-02: Failure of 120 Volt Vital AC Power Supplies.

Licensee documents DSN 0122790123 and DSN 1127795055 were reviewed.

Marble Hill does not use inverter static switches for automatic transfer

to alternate power supplies. This is accomplished by manual transfer

switches.

(Closed) IE Circular 79-11: Design / Construction Interface Problem.

Licensee documents DSN 0706790030 and DSN 0331804002 were reviewed.

The licensee determined that the NSSS-AE design interface is adequately

controlled. Westinghouse supplies Sargent and Lundy with specific

interface criteria to be incorporated into plant design. Westinghouse

vendor drawings are reviewed by Sargent and Lundy for proper interface

with all aspects of the balance of plant design. The licensee has not

identified any interface problem trends.

(Closed) IE Circular 79-12: Potential Diesel Generator Turbocharger

Problem. Licensee documents DSN 0706790029 and DSN 0725794022 were

reviewed. The licensee uses Colt-Pielstick PC2V diesel engines which

utilize a self-contained lube oil splash system for the turbochargers.

A pumped lube oil system is not used.

(Closed) IE Circular 79-13: Replacement of Diesel Fire Pump Starting

Contactors. Licensee documents DSN 0720790063 and DSN 0107805051 were

reviewed. The Cummins diesel fire pump engines identified in this cir-

cular are not used at Marble Hill.

(Closed) IE Circular 79-17: Contact Problem in SB-12 Switches on General

Electric Metalclad Circuit Breakers. Licensee documents DSN 0817790005

and DSN 1127795051 were reviewed. The licensee determined that General

Electric metalclad circuit breakers are not used at Marble Hill.

(Closed) IE Circular 79-19: Loose Locking Devices on Ingersoll-Rand

Pump Impe11ers. Licensee documents DSN 0920790102, DSN 1127795031,

DSN 0108805042, DSN 0211805032, and DSN 0313805021 were reviewed.

The licensee determined that all affected Ingersoll-Rand pumps used

at Marble Hill have the new modification, which provides a more posi-

tive mechanical lock on the impeller nut.

(Closed) IE Circular 79-20: Failure of GTE Sylvania Relay, Type PM Bulle-

tin 7305, Catalog SU12-11-AC with a 120V AC Coll. Licensee documents

DSN 0121805034, DSN 0212805039, and DSN 0214805008 were reviewed. The

licensee determined that the subject relay will not be used in safety-

related equipment or affect the operation of safety-related equipment.

(0 pen) IE Circular 79-22: Stroke Times for Power Operated Relief V11ves.

Licensee documents DSN 1126790021, DSN 1129795049, DSN 022580505),

DSN 1124804014, and DSN 1222803012 were reviewed. Prior to fuel load

the licensee will determine whether the periodic surveillance of the

power operated relief valves is necessary to assure that the valves will

stroke within two seconds. This circular will remain open pending that

determination.

(Closed) IE Circular 79-23: Motor Starters and Contactors Failed to Oper-

ate. Licensee documents DSN 1204790072, DSN 1129795048, and DSN 0214805009

were reviewed. The licensee determined that no Gould, Inc. NEMA Size 3

starters or contactors are planned for use at Marble Hill.

(Closed) IE Circulars 79-25 and 79-25A: Shock Arrestor Strut Assembly

Interference. Licensee documents DSN 0108805034 and DSN 0109804000 were

reviewed. The licensee determined that_these circulars are not applicable.

All mechanical shock arrestors are manufactured by, Pacific Scientific  !

Company and supplied through ITT Grinnell. These shock arrestors do not

have the interference probicms identified in this circular.

(Closed) IE Circular 80-01: Service Advice for General Electric

Induction Disc Relays. Licensee documents DSN' 0129805003, DSN 0128804003,

completion of the licensee's evaluation.

(0 pen) IE Circular 80-09: Problems with Plant Internal Communications.

Licensee documents DSN 0506805012, DSN 0515804008, DSN 0624805077,

DSN 0703805015, DSN 0802805049, DSN 0818804005, and DSN 1124804018 were

reviewed. Marble Hill will have public address, telephone, radio and

sound powered systems for internal plant communications. These systems

are capable of operation during loss of offsite power. Prior to fuel

load of Unit 1, the licensee will coordinate efforts in a survey to de-

termine all areas of the plant where radio transmissions might adversely

affect the operation of equipment. Measures will be taken to eliminate

or prevent any disruption of operation of equipment through posting of

signs, written procedures, or other methods that may be appropriate.

This circular will remain open pending completion of these ef forts.

(0 pen) IE Circular 80-10: Failure to Maintain Environmental Qualification

of Equipment. Licensee documents DSN 0506805009, DSN 0515804001, and

DSN 0702804012 were reviewed. This circular will remain open pending

completion of the licensee's evaluation.

(Closed) IE Circular 80-11: Emergency Diesel Generator Lube Oil Failures.

Licensee documents DSN 0520805020, DSN 0522804003, and DSN 0723805044

were reviewed. The licensee determined that no corrosion inhibitor will

be used at Marble Hill. Sulfuric acid will be added at the cooling tower

basins and pH will be carefully monitored and controlled. Solder is not

t used on the lube oil cooler tube-to-tube sheet joints. These tubes are

l mechanically cold-rolled into the tube sheet.

(Closed) IE Circular 80-12: Valve-Shaft-To-Actuator Key May Fall Out of

Place When Mounted Below Horizontal Axis. Licensee documents

i section will add to the valve test procedures checks for the valve shaft

to actuator key problem. These checks will be added to procedures CMTP-5,

l Manual Valve Functional Test; CETP-5, Motor Operated Valve Testing; and

CETP-14, Air Operated Valve Testing. Identified problems will be re-

solved prior to turnover to operations.

Circulation Cooldown. Licensee documents DSN 0624805000, DSN 0702804010,

DSN 1008804012, DSN 1010804009, DSN 1013804019, DSN 1028804003,

(Closed) IE Circular 80-16: Operational Deficiencies in Rosemount Model

510 DU Trip Units and Model 1152 Pressure Transmitters. Licensee documents

DSN 0707804011, DSN 0707804009, DSN 0723805045, DSN 0721805013,

DSN 0729804006, DSN 0829804000, and DSN 0821804000 were reviewed. The

licensee has determined that one Model 1152 Rosemount transmitter will

be used on each Hydrogen Recombiner System and environmental tempera-

tures greater than 175 F are not expected. The potential failure mode

identified in this circular occurs at environmental temperatures greater

than 175 F.

(Closed) IE Circular 80-17: Fuel Pin Damage Due to Water Jet From Baffle

Plate Corner. Licensee documents DSN 073,0804012, DSN 0801804015,

DSN 0819805001, and DSN 0926804007 were reviewed. Westinghouse deter-

mined that the core baffles in the Marble Hill units are designed such

that water jetting does not occur.

(Closed) IE Circular 80-21: Regulation of Refueling Crews. Licensee

documents DSN 0918804008, DSN 0918804000, and DSN 0924803002 were reviewed.

The licensee has reviewed the subject circular and will abide by the

regulatory requirements applicable to fuel handling activities.

(Closed) IE Circular 80-22: Confirmation of Employee Qualifications.

Licensee documents DSN 1009804003, DSN 1009804000, and DSN 1015804006

were reviewed. The Nuclear Division Project Personnel Department insti-

tuted a program which is consistent with the guidance in the circular.

The academic and nuclear-related experience of personnel who are engaged

in work which may be safety-related is verified by the licensee.

(Closed) IE Circular 80-23: Potential Defects in Beloit Power Systems

Emergency Generators. Licensee documents DSN 1107804022, DSN 1113804008,

and DSN 0102815002 were reviewed. The licensee determined that the frame

size for the Marble Hill c.ergency generators is such that the concerns

of this circular are not applicable.

(Closed) IE Circular 81-01: Design Problems Involving Indicating Push-

button Switches Manufactured by Honeywell Incorporated. Licensee

documents DSN 0128814007, DSN 0130814010, DSN 0130814009, DSN 0403815010,

DSN 0512814014, DSN 0608815136, and DSN 1228815141 were reviewed. The

licensee determined that the subject switches are not used at Marble Hill.

Sargent and Lundy will continue to monitor vendor documentation to ensure

that any possible future application of these switches are justified or

replaced with acceptable alternatives.

3. Evaluation of Items Reported Per 10 CFR 50.55(e)

(Closed) (546/81-05-EE; 547/81-05-EE) (Licensee designated number S 81-10):

Underrated Marathon Terminal Block in Limitorque Valve Motor Operators.

Licensee documents DSN 0724814000, DSN 0604815004, DSN 0611815016,

DSN 0727814005, DSN 0727814006, DSS 0805814000, DSN 0819814004,

DSN 0826814007, DSN 1130814004, DSN 1211814002, and DSN 0517825069

were reviewed. On July 24, 1981, the licensee notified Region III of

a potentially reportable deficiency regarding an underrated terminal

block which had been installed in several limitorque valve motor opera-

tors. The terminal block, Marathon 300 series, had originally been

rated for the specified service conditions of 600 volts and 30 amps,

but had been subsequently dorated to 300 volts and 30 amps. The licensee

submitted interim reports on August 24, 1981 and December 11, 1981. On

May 7, 1982, the licensee submitted a final report which stated that this

item is not reportable. Sargent and Lundy determined that the 480 volt

motor leads in these motor operated valves were only temporarily termi-

nated at the terminal blocks. These leads are normally connected directly

to field power cable when installed by the electrical contractor. There-

fore, no corrective action is required for the valve motor operators.

Sargent and Lundy has also surveyed Marble Hill project specifications

that identify Marathon 300 series blocks for their possible use by other

suppliers for applications greater than 300 volts and 30 amps. This

survey determined that safety related equipment suppliers used these

blocks for 120 volt control service, less than the revised lower ratings,

or did not use them at all. Equipment specifications are being revised

to eliminate the use of the subject block for greater than 300 volt and

30 amp service conditions.

4. Evaluation of Fire Prevention, Protection, and Suppression Programs

a. Portable Fire Extinguishers

The inspector toured the P-2 Warehouse, areas outside the containment

buildings, and the auxiliary building. Portable extinguishers were

, visually examined to determine if they were properly inspected, stored,

and maintained.

(1) P-2 Warehouse

All examined extinguishers were properly inspected, stored, and

maintained.

(2) Areas Outside the Containment Buildings

Three 150 lb Ansul wheeled extinguishers were examined. The inspec-

tion frequency could not be adequately determined due to weathered

inspection tags. Protective plastic extinguisher covers were damaged

or missing. The licensee agreed to use plastic covers over inspection

tags, use indelible ink for inspection tag signatures, and replace

the protective covers on wheeled exterior extinguishers.

(3) Auxiliary Building

The 20 lb Ansul extinguishers were stored in specially marked re-

fuse containers. Most inspection tags were weathered and wet and

several extinguishers were not positioned at designated locations.

In some cases extinguishers were covered with cement, which obscured

operating instructions. Some storage containers had accumulated

a large volume of water. Some extinguishers were removed from their

designated storage containers so that the containers could be used

for other purposes. The licensee agreed to place plastic covers over

the inspection tags and to use indelible ink for inspection tag

signatures. The licensee will investigate alternate methods for

storage of extinguishers. It appears that an increased extinguisher

inspection frequency is warranted in construction areas.

b. Welding Operations

During the tour of the auxiliary building, the inspector witnessed

five welding operations without a proper fire watch involving the

following contractors and locations:

The accompanying fire prevention inspectors stopped the welding operations.

A meeting was held afterwards by the fire inspectors with contractor safety

representatives to discuss the fire watch concerns. The licensee agreed to

place more emphasis on fire watches during welding operations.

c. Fire Prevention, Protection and Suppression Procedures

ANSI N45.2.3-1973, Paragraph 3.2.3, Fire Protection and Prevention,

requires the following:

(1) Instructions for the protection from, and prevention of, damage

by fire shall be provided in accordance with NFPA National Fire

Codes, Volume 4, Building Construction Facilities (1973);

(2) Procedures or instructions for fire protection shall include pro-

visions for fighting fires involving the use of available community

fire departments, trained project brigades and others;

(3) Procedures or instructions shall include plans for provision of

water supplies, hydrants, automatic sprinklers, access for fire

fighting, and distribution of extinguishers and fire fighting

equipment.

The inspector determined that the licensee did not have procedures

which implement the requirements of ANSI N45.2.3-1973, Paragraph 3.2.3.

The failure to implement procedures and instructions constitute noncom-

pliance with the requirements of Criterion V of 10 CFR 50, Appendix B

(546/82-11-01; 547/82-11-01). The licensee agreed to develop a

procedure implementing these requirements. '

5. Exit Interview "

. The inspector met with licensee representatives (denoted in Paragraph 1) -

and the NRC senior resident inspector en May 21, 1982. The inspector

summarized the scope and findings of the inspection. The licensee ac-

knowledged the findings.
