BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - RESPONSE TO VIOLATION
- 50-438,50-439/82-03 FAILURE TO ACHIEVE REQUIRED QA TRAINING OF CRAFT SUPERVISOR PERSONNEL This is in response to F. S. Cantrell's letter dated January 26, 1982, report numbers 50-438/82-03, 50-439/82-03, concerning activities at the Bellefonte Nuclear Plant which appeared to have been in violation of NRC regulations. Enclosed is our response to the citation.

1 ENCLOSURE BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 FAILURE TO ACHIEVE REQUIRED QA TRAINING OF CRAFT SUPERVISOR PERSONNEL SEVERITY LEVEL V VIOLATION 50-438,50-439/82-03-01 Description of Deficiency 10 CFR 50, Appendix B, Criterion II, as implemented by FSAR section 17.1 A.2, requires the licensee's QA program to provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained.
Quality Control Procedure (QCP)-10.30, Revision 2, Section 5.4.1, states that the Construction Engineer designates and shows on Attachment C those procedures in which each craft (craft superintendents, assistant craft superintendents, and active hourly foremen) will be trained.
Contrary to the above, training records examined for a protective coatings active hourly foreman (Badge No.10-040) revealed that he had received training in only two of the five required procedures listed on Attachment C for Painters. Examination of additional craft supervisory training records and discussions with responsible site personnel revealed that this deficiency is not limited to painter craft supervision personnel only.
Admission or Denial of the Alleged Violation TVA admits the violation occurred as stated.
Reason for Violation The violation occurred as the result of failure to follow Quality Control Procedure BNP-QCP-10 30, Craft Quality Assurance Training. The active hourly foreman (Badge No.10-040) was acting in a supervisory position for a process in which he was not trained as required by QCP-10 30.
Corrective Action Taken and Results Achieved All applicable craft superintendents and assistant craft superintendents have been reinstructed by memorandum dated March 23, 1982 of their direct responsibility to ensure that each foreman and dual-rated foreman under his supervision has received and maintains the proper training of applicable QC procedures. The deficient painter foreman cited in this report has been trained in the process (BNP-QCP-2.4) which he was directing. The training was performed on March 12, 1982.
Steps Taken to Avoid Further Violations Craft superintendents and assistant craft superintendents shall thoroughly review training records of each foreman and dual-rated foreman under his supervision to ensure that each individual has been trained to the applicable procedures. They will also closely monitor craft training to ensure compliance with the craft training procedure requirements.
