Dear Mr. Engelken:

On August 23, 1977, the NRC issued a Notice of Violation to PGandE identifying a construction activity at Diablo Canyon not in full L

compliance with NRC requirements. In accordance with 10 CFR 2.201, this i

report states the action taken to correct the discrepancy and to prevent recurrence.

The Notice of Violation was issued because pipe-rupture re-straint-steel field weld documentation was found not to conform to the contractor's specification. Two specific items of nonconformance that involve field weld documentation are listed in the notice. The first addresses Field Weld FW-40 and points out that the root pass was not I inspected as evidenced by "NA" on the process sheet. The contractor's specification ESD-243, paragraph 2.5.1, references American Welding Society, Dl.0-69, and lists seven inspection points such as fit-up, preheat, root pass and final inspections. These inspection points are intended to be applied only as required by AWS Dl.0-69 and PGandE I

Specification 8833XR. Neither AWS Dl.0-69 nor 8833XR require root pass i

inspection on fillet welds such as FW-40; therefore the "NA" is approprs ate on the process sheet. The contractor's specification ESD-243 has been changed to clarify when the listed inspection points are applicabic-

All process sheets for Unit 1 and 2 pipe-rupture restraint welds have been reviewed. Subsequent to your audit, the review of Unit 1 documentation and neces-sary corrective actions were completed.

The contractor, Pullman Power Products, has held meetings with his inspectors to emphasize specification requirements and the importance of proper inspection documentation. We believe these actions are adequate to prevent recurrence of the violation.
