J AFFIDAVIT OF ANTHONY N. FASAN0 I, Anthony N. Fasano, being duly sworn do depose and state:

I am employed as the Chief TMI Resident Section of the Region I Office.
U.S. Nuclear Regulatory Commission. A copy of sty professional qualifica-tions is attached to this affidavit.

The purpose of this affidavit is to provide a description of my respon-sibilities in connection with the preparation of a document entitled
" Recommendations of TMI-21E Investigation Team (Operational As sets l" ~ ~ -'
dated September 1979 and to describe how the NRC Action Plan (NJREG-0660) deals with certain of these recomendations. Specifically, the affidavit deals with those recomendations cited by intervenors, Union of Concermed Scientists and Stephen C. Sho11y in their motions to reopen the record filed September 10,1981 ("Intervenors' Motion").

In April of 1979, I was assigned to the TMI-2 Investigation Team in the l
Office of Inspection and Enforcenent. The purpose of the team was to conduct a fact-finding investigation concerning the March 28, 1979, accident.
I participated in the investigation es a tea.m member to this end. The results were published as NOREG-0600, 4.
After completion of NUREG-0600, I, as a team member, participated in the preparation of a memorandum containing views with regard to technical issues that arose during the course of the investigation. The memoranda,

were not intended as final NRC staff conclusions ready for implementation.

Based on discussions with NRC Staff persons, personal review of NUREG-0660, NUREG 0737, additional documents and testimony, it appears that the team recccrendations, under contention, raised by the intervenors, Union of Concerned Scientists and Stephen C. Sho11y were addressed during the fonnu-lation cf NUREG-0C60 and 0737.

I am,the Section Chief of the TMI Resident Section responsible for supporting the NRC activities on-site at Three Mile Island and for implementing the Off' ice of Inspection and Enforcement progress of inspections at TMI.
I have a Masters Degree in Chemical Engineeripg from New York University.
I From 1953 to 1958 I was employed at Brookhavein National Laboratory Graphite Pile Reactor, Upton, New York.
I w s employed as a Technical Specialist and as a design engineer in equipment fbr radiation research.. From 1958 to 1971 I was employed at Wright Pattersnn Air Force Base.
I served as a Project Engineer, Supervisor Engineer, Brahch Chief 'and finally as Division Chief at the Wright Paterson Nuclear Faci)1ty.
