This letter forwards the report on the interfacing system loss-of-coolant

I

(ISLOCA) inspection performed by an NRC team during the period from April 16

through April 27, 1990 at the Catawba Nuclear Power Station.

The inspection

l

was conducted by members of the Office of Nuclear Reactor Regulation,

,

-Region II, and NRC contractors.

At the conclusion of the inspection, we

discussed the findings with members of your staff identified in the enclosed

'

inspection report.

The primary objective of this inspection was to evaluate specific plant design

features, systems, equipment, procedures, operation activities, and human

actions which could affect the initiation or progress of an ISLOCA. This

included identification of generic events or system features related to

ISLOCAs, potential initiating or precursor events, as well as potential human

performance and human errors.

,

The team found that, in general, the interfacing systems at Catawba Nuclear

,

Power Station were adequately maintained and tested to prevent gross-failure of

i

pressure isolation valves which could initiate an ISLOCA event. Additionally,

the team did not identify any significant deficiencies in the area of-

man-machine interface which could significantly increase the probability of an

operator error initiating an ISLOCA event. However, the inspection team had

additional specific findings in the areas of: (1) operators recognition of

ISLOCA phenomenon;-(2) available alarms which were available to warn the

operators of an ISLOCA; and (3) procedures used for remote shutdown which had

incomplete or missing caution statements indicating that the interlocks for the

decay heat removal system were bypassed.

Although not addressed in the report, on June 11, 1990, failure to follow

l

procedures led to valve misalignment which resulted in an ISLOCA precursor

'

event. The event, along with other items, will be discussed between Duke Power

and NRC in a meeting scheduled for June 18, 1990.
