1.0 PURPOSE Upon activation of the Corporate Emergency Response Plan, this procedure identifies individuals assigned key roles in the Corporate Emergency Reeponse Organization and describes their notification and work assignment.
2.0 APPLICABILITY This procedure applies to Corporate Emergency Response Organization personnel 3.0 RESPONSIBILITIES 3.1 Emergency Response and Recovery Director
- Initiate and coordinate the notification process. ,
o Communicate and coordinate with the Emergency Coordinator to apply corporate manpower and resources as necessary.

- Ensure notification of appropriate personnel. ( Attachment 3 provides a listing of all corporate personnel available for response).
3.2 Emergency Suocort Manager
- Ensure notification of appropriate personnel.
- Coordinate with Technical and Engineeri, g Suppcrt Supervisor to assign personnel to use the status boar < ; and function as l communicators.
3.3 Support Services Coordinator
- Ensure notification of appropriate personnel.
3.4 Senior Vice President. Energy Resources and Utilization, Research and tnvironment
- - Ensure notification of appropriate personnel .
3.5 Radiological and EOF Manacer
- Ensure notification of appropriate personnel .

- Coordinate with the Emergency Support Manager to assign personnel to use the status boards and functional as communicators.
3.7 Secretary Nuclear Generation Division e Ensure notification of appropriate personnel .
- Collect notification logs after notification is completed.
o Assume the function of a historian and ensure that all necessary records are maintained and that important events are recorded.
4.0 INSTRUCTIONS 4.1 The Emergency Response and Recovery Director shall initiate notification ,
of key individuals of the Corporate Emergency Response Organization as f's l
follows:
4.1.1 Determine individuals to be contacted.
The particular individuals to be contacted within the Corporate Emergency Response Organization will depend upon the event and classification. Attachment 1, " Emergency Response &ganization Mobilization Matrix," is to be used as guidance to identify those individuals. The matrix is used as follows:
- a. Identify Emergency Action Level (obtained from DAEC).
- b. Use matrix to determine individuals to be contacted (blank box indicates no contact).
- c. Determine required action to be included in message (from synbol legend) .
- d. If activation of the EOF is required, the Emergency Support Manager will coordinate with the Technical and Engineering Support Supervisor to ensure personnel are assigned to fill the following functions:

4.1.2 Contact individuals.
The primary means of notification should be via remote beepers (pagers) or by telephone. Individuals w:.o carry beepers are identified throughout Attachment 3. The Emergency Response and Recovery Director or his designee shall conduct the initial notifications.
- a. The general, overall flow of Notification is illustrated in Attachment 2.
NOTE The Emergency Response ar.d Recovery Director initiates several " chains" of notification which s' auld occur in parallel.
- b. Attachment 3 is a combination telephone list and notification log and should be used to identify personnel available, and record when notification takes place.
O c. Allow at least 10 rings (about one minute) for a call to be answered. If there is no response, move on to the alternate contact.
- d. If, for any reason, a primary and an alternate cannot be reached for a position which is in turn responsible for further contacts, the individual attempting contact shall assune responsibility for those succeeding notifications.
For example, if the Emergency Response and Recovery Director cannot reach the Radiological and EOF Manager or his alternate, the Emergency Response and Recovery Director shall assume the notification responsibilities of the Radiological and EOF Manager. In addition, his designee shall continue efforts to reach the primary and/or alternate ccntacts until notification is completed.
4.2 Notification message content should be prepared as follows:
4.2.1 Message content should be determined based on Attachment 1.
- a. Attachment 4 should be used for standby status notification,
- b. Attachment 5 should be used for the mobilization

,q Date - 12/23/81 wj 4.3 Notification contacts should be confirmed as follows:
4.3.1 Each individual with notification responsibilities should maintain a Notification Log Sheet (Attachment 3).
4.3.2 Temporary reassignment of responsibilities may be necessary if a primary and an alternate contact for an emergency response organization position is initially unavailable. These reassignments will be as directed by the Emergency Response and Recovery Director.
4.3.3 The Emerggncy Response and Recovery Director or his designee shall monitcr the status of notification periodically until all personnel are at their assigned locations. The Emergency Response and Recovery Director or his designee will be contacted by individuals with notification responsibilities to report the status of their Notification Log and determine problem areas.
The Notification Logs shall be signed off and submitted to the Secretary, Nuclear Generation Department when notifications are complete.

Revision 1 EMERGENCY NEWS CENTER ACTIVATION Date - 1/14/82 b-1.0 PURPOSE Upon activation of the Corporate Emergency Response Plan (The Plan) this procedure provides instructior.s for activating the Emergency News Center in response to an emergency situation at DAEC.
2.0 APPLICABILITY Upon activation of The Plan this procedure applies to members of the Corporate Emergency Response Organization involved with the Emergency News Center.
3.0 RESPONSIBILITIES 3.1 Emergency Response and Recovery Director
- Notify the Emergency News Center Director 3.2 Emergency News Center Director c

- Direct and coordinate activation of the Emergency News Center
- Notify the Corporate Aff airs staff, Corporate Services, and the media of the emergency 4.0 INSTRUCTIONS 4.1 Upon Activitation of The Plan the Emergency News Center shall be activated as follows:
- During a notification of an Unusual Event or an Alert, the Emergency News Center is activated at the discretico of the Emergency News Center Director.
- During a Site or General Emergency, the Emergency News Center is immediately activated.
4.2 The Emergency Response and Recovery Director shall notify the Emergency News Center Director (refer to CPIP 1.2, " Personnel Assignment and Notification").

4.3 Emergency News Center Director shall respond as follows:
4.3.1 Initially notify the media using a brief, preplanned news release (refer to Attachments 1 and 5, " Emergency News Center Notification Checklist" and " Preplanned News Releases"). The news releases shall be handled as follows:
- a. For an event of minor significance, where escalation is not anticipated (occurring during regular work hours or off-hours), the release will be issued during the workday as routine business,
- b. For a major event (occurring during regular work hours or off-hours), the release will be issued as soon as possible following evaluation of the extent and tragnitude of the event by the Emergency Response and Pecovery Director.
O 4.4 The Emegency News Center Director shall notify:
- Assistant Directors of the Corporate Aff airs staff to report to the Emergency News Center. (Refer to Attachment 1,
" Notification Checklist").
- Support Services Coordinator to coordinate preparation of the Emergency News Center for emergency use. (Refer to Attachment 1, " Notification Checklist").
4.4.1 The Assistant Directors shall, in turn, notify the Corporate Aff airs personnel who shall be instructed to report to the lith Floor.
4.4.2 The Nuclear Information Coordinator or designee, using Attachment 1, " Notification Checklist," shall contact outside agencies regarding the emergency situation. The public relations consultant by contract, shall provide 6 or 7 communications specialists, as necessary, to assist with Emergency News Center activities. The specialists shall be instructed to recort to the 6th Floor.
4.5 The Emergency News Center Director shall organize and direct preparation of the Emergency News Center for use as follows:
4.5.1 Based on an estimate of the ntsnber of rews people Qw present and/or. anticipated, determine if the existing 6th floor facilities are adequate.

- a. The 6th floor of the IE Tower shall be the primary location for the Emergency News Center if less than 200 members of the press are anticipated.
- b. The Paramount Theater shall be the backup location for the Emergency News Center and shall be activated if more than 200 members of the press are anticipated.
This will take place if the declared EAL has the potential for national or international media attention.
- c. If an event escalates and the need arises, the news briefings shall be held at the Paramount Theater.
4.5.2 During normal working hours, direct IELP personnel who normally work on the 6th floor to evacuate the area.
4.5.3 Complete a work assignment sheet to delegate certain responsibilities and work tasks for corporate aff airs and f')
d supplemental personnel for continuous manning during activation of the Emergency News Center (refer to Attachment 3, " Sample Work assignment Sheet for an 8-hour Shift Team").
4.5.4 Interf ace with the Support Services Coordinator to prepare the following areas of the Emergency News Center for use and to provide the manpower to maintain these areas while the Emergency News Center is activated:
- a. The 6th floor, which consists of the Emergency News Center Director's office, public information and display area, briefing room, press and media workroom, and dining room and kitchen (refer to Attachment 4,
" Emergency News Center IE Tower").

- b. The lith floor, which the Corporate Affairs Staff, I clerical support, and supplemental staff shall occupy.

4.5.5 Supervise completion of the following tasks (use Attachment 2 as a guide to complete these tasks).
- a. Move office equipment into the Director's office and additional supplies into the nearby storage room.
- b. Provide additional equipment and supplies required on the lith floor,
- c. Establish and verify communication between the Emergency News Center end the Emergency Operations Facility, and between the 6th and lith floors.
- d. Provide oeepers to the Emergency News Center Director and his assistants for use when away from the Emergency News Center.
- e. Provide equipment and supplies for the press in the Press and Media Work area.
- f. Obtain additional plug-in telephones which can be added to various areas, as needed, through preinstalled telephone jacks.
- g. Set up reception desk.
- h. Set up Public Information and Display areas.
- i. Locate additional required audio-visual equipment and store in the audio-visual room behind the auditorium stage.
4.5.6 Verify that security has been established at the 6th floor IE Tower elevators.
- a. The Security Director shall assign a guard to the 6th floor to enforce access controls to the Emergency News Center (refer to CPIP 1.5 " Building Security Implement ation") .
- b. A receptionist shall be assigned to perform the following tasks:

- Maintain a log of all individuals entering and exiting the area.
- Verify identification and issue registration badges for news media and other personnel authorized access to the 6th floor.
- Maintain registration forms for each member of the press and personnel other than IELP employees.
- Spot check identification following initial badging.
- Hand out instructions to the media.
4.6 Emergency News Center Director, upon verification that the O- Emergency News Center is operational, shall:
- Announce the opening of the Emergency News Center to the media i
(use Attachment 1 " Notification Checklist" as a guide).
- Inform the Cedar Rapids Police Department of possible traffic congestion at the IE Tower due to TV trucks, lights, etc.
I 4.7 Deactivation ,
4.7.1 The Emergency Response and Recovery Director shall direct the Emergency News Center Director to deactivate the Emergency News Center under the following conditions:
- a. The plant has been placed in safe and stable condition,
- b. Media interest diminished and full facilities are no longer necessary.

4.7.2 Emergency News Center Director sh'all direct deactivation of.

- a. Instructs the Support Services Coordinator to remove extra communications equipment and office equipment and supplies. Attachment 2, " Equipment and Supplies Checklist" should be used as a guide.
- b. Issue follow-up information to the news media (if necessary) from the lith floot of the IE Tower.
- c. Prepare a written report for the Emergency Response and Recovery Director which shall include the following information:
- Summary of events during activation of the Emergency News Center.
- Effectiveness of the Corporate Emergency Response Plan relating to media coatact and suggested

this procedure describes the actions to be taken to implement building security at the ie: Tower.
2.0 APPLICABILITY Upon activation of The Plan, this procedure applies to the IELP Security Director.

- 1. Linn County Sheriff's Department and Cedar Rapids Police Department will be primarily responsible for county emergency response functions.
- 2. Additional security personnel may be available from the Wackenhu Security Agency which provides contract security services to the DAEC.
4.1 Immediate Action 4.1.1 Upon notification by the Support Services Coordinator that an emergency situation exists, the Security Director shall:
- a. During normal working hours (6:00 a.m. - 6:00 p.m. ,
Monday through Friday), provide for a building-wide announcement as prescribed in Attachment 7 to be made by the IELP switchboard operator instructing personnel O to take the following actions:
- All IELP and contract personnel assigned to the ie:
Tower shall report to their normal work location.
- All industry representatives shall vacate Iowa Electric portions of the building unless specifically requested to remain by their IELP cont act.
- All visitors shall leave Iowa Electric portions of the building,
- b. During off-normal hours (6:00 p.m. - 6:00 a.m. ,
weekends, holidays), instruct the guard stationed at the 1st floor elevator lobby that no one shall be allowed access to the elevators until all security posts are established. The exception to this will be IELP personnel with an IELP picture identification card.
- c. Request manpower from the agencies listed in Attachment 2 with whom IELP has established contract agreements.
A minimum of five guards should be on duty at all times.
O . Guares should be instructed to report to the 1st floor elevator lobby of the ie: Tower in their agency uniforms.

o Assist in escorting all unauthorized personnel to the elevators.
- Log all other personnel into the Personnel Access Log ( Attachment 4) .
- e. Notify the ie: Tower Building Engineer, Towle Management Corporation, of the emergency situation. He will provide notification to other building tenants.
- f. Lock stairwell doors on 6th, 14th, and 20th floors.
- g. Meet security guards reporting to the 1st floor elevator lobby.
e Assign security posts (see Attachment 1).
o Provide instructions regarding their duties and responsibilities, e Distribute two-way security radios to each guard.
- h. Establish security posts in the following order of priority; o ist floor elevator lobby.

1.0 PURPOSE Upon activation of the Corporate Emergency Response Plan (The Plan),
this procedure describes the range of IELP corporate technical support services which are available in response to an event at the OAEC, including mobilization and coordination of the technical support staff.
2.0 APPLICABILITY Upon activation of The Plan, this procedure applies to the Emergency Support Manager, the Technical and Engineering Support Supervisor and members of the corporate engineering staff.
3.0 RESPONSIBILITIES 3.1 Emergency Support Manager
- Provide direction and assure proper execution of requests for O techaicei, ea9 4neerin9 and support services
- Coordinate activities, resources and capabilities of IELP support organizations and response groups to perform tasks in support of the overall response and recovery effort 3.2 Technical and Engineering Support Supervisor
- Provide direction and assure proper execution of all requests for technical and engineering services from the Emergency Support Manager.
4.0 INSTRUCTIONS The following paragraphs provide instructions for technical support coordination. Since the need for technical support is dependent upon the nature of the emergency, these instructions are general guidelines to assist the Emergency Support Manager and the Technical and Engineering Support Supervisor in providing technical assistance to achieve stable plant conditions.
- CPIP 1.2 " Personnel Assignment and Notification" Attachment 3 provides the names and telephone numbers of key contacts and backups within IELP engineering, design, and construction.

4.1 The Technical Support Group shall be mobilized as follows:
4.1.1 Upon activation of The Plan for notification of an Unusual Event or an Alert:
- a. The Emergency Support Manager and the Technical and Engineering Support Supervisor may be placed on standby as described in CPIP 1.2 " Personnel Assignment and Notification" e The Emergency Support Manager and the Technical and Engineering Support Supervisor, based upon the plant conditions and character of the event, shall determine the individuals. in the Technical Support Group, to be placed on standby.
- The Technical and Engineering Support Supervisor shall contact the personnel determined above using Attachment 1.
- b. If the emergency classification is escalated to a higher level, notification and mobilization of support perscnnel shall be as prescribed in the next subsection.
. c. When the condition or situation has been resolved, the l Technical and Engineering Support Supervisor shall remove all individuals contacted from standby status.
4.1.2 Upon activation of The Plan for Site and General Emergency:

4.2 The staff shall be augmented as follows:

- a. The Emergency Support Manager shall inform the Technical and Engineering Support Sueprvisor of the request for staff augmentation. Types of augmentation include personnel who will participate in:

- b. Using CPIP 1.2 Attachment 3, the appropriate individuals should be chosen by the Technical and Engineering Support Supervisor.
4.2.2 Tracking and Expediting Response Activities
- a. Individuals contacted by the Technical and Engineering Support Supervisor or the Emergency Support Manager to support the staff shall, at the time of contact, be provided the following information:
- The nature of assistance required.
- The operation to which they report (i.e., Emergency Operations Facility, Technical Support Center, etc.).

- The person to whom they report.
- b. The Technical and Engineering Support Supervisor shall maintain records which include the individuals dispatched, their locations, and supervisors.

4.3 Engineering and Design Assistance shall be Provided as Follows:
4.3.1 Near-term activities If the request for assistance from the DAEC involves activities critical to establishing stable plant conditions, normal administrative controls may not be employed. The Ecergency Support Manager shall coordinate with the Manager, Corporate Quality Assurance to ensure that Nonconformance Reports are written for ai; emergency activities performed during an emergency condition and the Nonconformance Reports are identified as emergency-related.
- a. Task Identification
- 1. The Emergency Support Manager shall inform the Technical and Engineering Support Supervisor of the g request for engineering and design assistance.

- Emergency design changes Such changes shall be conducted in accordance with IELP's Administrative Control Procedure
" Emergency Design Changes."
e Procurement activity Procurement activity shall be coordinated through the Emergency Support Manager in accordance with CPIP 2.3, " Administrative Services and Corporate Response Coordination."
- Vendor contact Vendor contact shall be conducted with the approval of the Emergency Support Manager by the Technical and Engineering Support Supervisor or his designee.
- Procedure development Procedure development shall be conducted using O itte s ^e=iaistrative coatroi erocedures; Procedure Format, Preparation and Control of Permanent Revisions and Control of Temporary Revisions to Procedures. Normal administrative controls shall be handled as discussed earlier.

- b. Tracking and expediting response activities as described in paragraph 4.2.2 of this procedure 4.3.2 Long-term Activities Once stable plant conditions have been achiend, technical activities are carried cut as they would be under normal circumstances.
All activitics performed under emergency procedures which require formal docunentation must be followed up.
4.4 Construction Assistance shall be orovided as follows:
4.4.1 Near-term Activities Near-term construction activities will consist of installation of temporary modifications or repairs.
- a. Task Identification Once a need for modifications has been established, the Technical and Engineering Support Supervisor shall arrange for construction and schedule the work. If sufficient manpower is not available from wihtin the station maintenance personnel group, the Technical and Engineering Support Supervisor will select appropriate individuals / firms from Attachment 1 for assistance,
- b. Tracking and Expediting Response Activities as described in Paragraph 4.2.2 of this procedure.
4.4.2 Long-term Activities Long-term construction activities take place once stable plant conditions have been established. These activities include construction and contract administration for design changes. The control of these activities should be carried out as described in normal administrative control procedures.

2.0 APPLICABILITY Upon activation of The Plan, this procedure applies to the Emergency Response and Recovery Director and corporate management.

This form should be filled out prior to contacting a Federal, State, or local official, and should be sequentially numbered.

e The log book serves two purposes.
- Provides a location to file the completed Emergency Update Message Form.
Provides an historical record of the information provided, to whom it was provided, and when it was provided.
e The log book will be maintained under the direction of the Emergency Response and Recovery Director.
4.1.3 When the condition or situation has been resolved:
- a. A closecut Message Form (Use Attachment 3 as a guide) should be completed with all pertinent data. The Closecut Message Form includes a summary sheet which O should be filled out for each verbal comunication that has been conducted during the event.
- b. Once the appropriate individual has been contacted, the completed Closecut Message shall be filed in the communications log book.
4.2 Upon activation of The Plan the following actions shall be initiated:

- The Emergency Response and Recovery Director shall determine which of the individuals listed may be contacted.
4.3.2 Follow-on Activities
- a. When a press release is made, persons identified above should be re-contacted as appropriate and apprised of its contents prior to its release.
n b. State and Local E0C's will receive updates hourly or as V conditions warrant, from the Radiological and EOF Manager,as perscribed by the Emergency Response and Recove y Director,
- c. When the condition or situation has been resolved, a verbal summary (see Attachment 3) to all government and support agency officials notified will be made by the Emergency Response and Recovery Director or his designee.

CORPORATE PLAN IMPLEMENTING PROCEDURE CPIP - 5.1 Page 1 of 8 EMERGENCY RESPONSE AND REC 0VERY DIRECTOR Revision 0 Date - 12/11/81 1.0 PURPOSE 1.1 This procedure describes the actions required by the Emergency Response and Recovery Director during an emergency situation at the Duane Arnold Energy Center.
2.0 APPLICABILITY 2.1 This procedure applies to the Emergency Response and Recovery Director.
2.2 The Director-Nuclear Generation is the primary individual designated to assune this position upon activation of the Corporate Emergency Response Organi zation. The designated alternates are the Assistant Vice President-Nuclear Generation and the Manager Mechanical and Nuclear Engineering.
3.0 RESPONSIBILITIES 3.1 General Responsibilities _
O 3.1.1 The Emergency Response and Recovery Director is accountable to the Assistant Vice President-Nuclear Generation and is respnnsible for the overall direction and control of the integrated Emergency Response and Recovery effort. He will inform Corporate Management of those matters which impact on the safe recovery of the station and may affect the general public.
He has the full authority and responsibility to make commitments for the Company related to emergency response and DAEC recovery activities in the event of an Alert, Site Emergency or General Emergency. This authority is granted until the plant is in a safe and stable condition and the emergency classification is terminated.
3.1.2 The Emergency Response and Recovery Director shall coordinate the efforts of the Radiological and EOF Manager and the Emergency Support Manager with the Emergency Coordinator.
3.1.3 The Emergency Response and Recovery Director shall approve the protective action recommendations to off-site authorities made by the Radiological and E0F Manager. He must ensure that the protective action recomendations are based on the Emergency Coordinator's analysis of current trends in degrading reactor core conditions as well as potential or actual radiolo ical release conditions in order to identify potential prob ems.

3.1.4 The Emergency Response and Recovery Director shall ensure that the Emergency Response Organization is staffed by qualified personnel and that sufficient support for various functions are avail able either from within IELP or from outside organizations (other utilities, Architect Engineers, Nuclear Steam Supply System Suppliers, consultants, etc.).

4.0 INSTRUCTIONS 4.1 Actions Required on Initial Notification of a Declared Emergency Action Level (EAL).
4.1.1 The Director-Nuclear Generation (or his alternate) will receiv e initial notification of a declared Emergency Action Level at the DAEC from the Emergency Coordinator. He shali then review the sequence of events, the logic used to establish the EAL and confirm the declared EAL.
4.1.2 The Director-Nuclear Generation, based on CPIP 1.1, " Corporate Emergency Response Plan Activation," shall determine his next course of action as follows:
- a. Upon notification of an Unusual Event or an Alert, initiate one of the following courses of action based on the current situation and projected trend:
O (1) Inform eparapriete Iste personnei of the situetion end, if appropriate, place them on standby (use Attachment 1 of CPIP 1.1 as a guide). Standby does not constitute activation of the Corporate Emergency Response P1 an.
Those individuals placed on standby have no further action, other than to keep appropriate IELP management apprises of their location (2) Activate the Corporate Emergency Response Plan. Thi s invokes the Corporate P1 an Implementing Procedures 1.2 through 3.2.
- b. Upon notification of a Site or General Emergency, initiate immediate activation of the Corporate Emergency Response Pl an . Simultaneously, the Director-Nuclear Generation assunes the position of Emergency Response and Recovery Director.
4.2 Mobilization of the Emergency Response Organization 4.2.1 The Emergency Response and Recovery director shall mobilize the Emergency Response Organization using the mobilization matrix, Attachment 1 of CPIP 1.2, " Personnel Assignment and Notification."

4.2.3 The Emergency Response and Recovery Director shall receive reports from the Emergency Coordinator and the individuals listed above when the notification process is complete and the emergency f acilities are fully operational.
4.3 Notification of Outside Agencies 4.3.1 Upon activation of the Corporate Emergency Response Plan, the Emergency Response and Recovery Director shall assune the responsibility of informing the NRC Region III Director and the Director of Nuclear Reactor Regulation of the changes in the emergency condition at DAEC.(CPIP 3.1 Attachment 1) 4.3.2 Attachment I, " Government Officials and Support Agency Contacts" of CPIP 3.1, " Government Relations," lists additional Federal, State and Local Officials.as well as support agency management personnel that may be contacted as directed by the Emergency Response and Recovery Director.
C 4.4 Continuing Actions Required From the Emergency Response and Recovery Director 4.4.1 Brief the E0F staff at frequent intervals to' ensure that the latest conditions at he DAEC are understood and projected trends or potential problems are considered.

4.4.3 Ensure follow-up notifications to the NRC are made as the situation changes.
- 4.4.4 Ensure that the Radiological and E0F Manager is giving the State and local E0Cs an update of the situation as conditions warrant, or hourly at a minimum (CPIP 3.1, " Government Relations").
4.4.5 Review for technical accuracy any press releases prior to their dissemination, verify the initiating event sequence (if known) with the Emergency Coordinator and provide the Emergency News Center Director with accurate and timely information on all subsequent events (CPIP 3.2, " Media Briefings and Public Relations").
4.4.6 Ensure that the Emergency Support Manager has informed the Insurance and Risk Manager and the Safety Director of personal p injury accidents-(CP-IP 2.5, " Insurance and- Cl-ims" and- CPIPT.7, "S afety" ) .
4.5 Downgrading EALs and Deactivation of the Emergency Response Organization 4.5.1 Consult with the Emergency Coordinator to determine what portions of the Emergency Response Organization may be deactivated. EPIP 5.1, " Deactivation of the Emergency Response Organization" lists the following guideline:
An emergency which results in the activation of the E0F and supporting outside agencies shall be declared "under control" or "over" when the plant is in a stable state, necessary plant operating equipment is functioning properly and there is no potential for uncontrolled radiological releases.
4.5.2 CPIP 3.1, " Government Relations," requires a verbal summary to all government and support agency officials notified when the emergency condition has been resolved. Within eight (8) hours of the verbal summary, a written stenmary shall be sent to the NRC and other agencies as appropriate for a General Emergency, Site Emergency or an Alert. An unusual Event requires a written summary within twenty-four (24) hours of the verbal sumary. At the discretion of the Emergency Response and Recovery Director, it may also be sent to each of the individuals contacted during the emergency.

2.0 APPLICABILITY This procedure shall be implemented upon declaration of an event classified as an Alert, Site Emergency or General Emergency. The Emergency Coordinator, at his discretion, may implement this procedure and activate portions of the Emergency Response Organization during a situation classified as an Unusual Event.
3.0 RESPONSIBILITIES 3.1 Coordinate accident assessment and analysis efforts with the Radiological and E0F Manager to determine the potential or actual radiological impact of the emergency conditions upon the public.
3.2 Coordinate efforts with the Emergency Support Manager, as necessary, to n'- return the plant to a stable safe condition.
3.3 Ensure coordination between the plant staff and the corporate emergency response organization.
3.4 Direct and coordinate the inplementation of plans, procedure and schedules to meet recovery operation. Recovery represents that point following an emergency condition when the plant is in a stable state, necessary plant operating equipment is functioning properly and there is no potential for uncontrolled radiological releases.
3.5 Where alternate courses of action exist, decisions as to which course to follow will be directed by the Emergency Coordinator.
4.0 INSTRUCTIONS 4.1 Upon declaration of an event classified as an Alert, Site Emergency or General Emergency the Emergency Coordinator shall proceed immediately to the Control Room and review with the SSE or Control Room Coordinator:
4.1.1 The status of the plant.
4.1.2 The logic used to establish the classification of the event.
4.2 Assure activation of the TSC and OSC upon declaration of an Alert, Site O- Emergency or General Emergency. During an Unusual Event the TSC or OSC may be activated if deemed necessary by the Emergency Coordinator.
4.3 Assure that initial notification has been made to offsite agencies, necessary response organizations and that continuous communications with the NRC has been established.

b) Evaluate the effects of abnormal systems configeration on future operational evolutions.
c) Ensure that technical evaluations are being conducted with the most current information and that operational evolutions are properly planned.
4.5 Notify the Director, Nuclear Generation or his alternate and review:
4.5.1 Emergency Action Level and the basis for its selection.
4.5.2 Plant status and actions being taken to mitigate the condition.
4.5.3 Sequence of events to the extent understood.
4.6 Evaluate changes in the plant status for potential radiological consequences to the public and apprise the EOF of all such changes and the results of evaluations conducted.
4.7 If the E0F is not yet manned provide plant status to County, State and NRC representatives as appropriate. Protection action recommendations shall be provided to offsite agencies and shall consider:
4.7.1 Degrading reactor core conditions.
4.7.2 Radiological release conditions.
4.8 Review the attached checklist and assure that the actions specified therein are being taken.

2.0 APPLICABILITY This procedure applies to Emergency Response Organization Management after the plant is restored to a safe and stable condition or when the Recovery Phase is initiated. Deactivation may be performed as appropriate in one l complete step (for minor events) or may be carried out in several steps as emergency functions are no longer needed or are scaled down.
3.0 RESPONSIBILITIES 3.1 Emergency Coordinator 3.1.1 For a Notification of Unusual Event where the Emergency Os Operations Facility was not activated, initiate steps to l deactivate the Site Emergency Response Organization. I 3.1.2 For other Emergency Action Levels, consult with the &nergency Response and Recovery Director and make recommendations to deactivate portions of the Site Emergency Response Organization and any of its functions af ter the Recovery Phase begins.
3.1.3 Notify the Emergency Response and Recovery Director when the l plant has achieved a safe and stable operating condition.
3.1.4 Coordinate the deactivation of the Site Emergency Response Organization and the return of site personnel to their normal duties and work schedules.
3.1.5 Coordinate the notification of local, State and Federal support agencies regarding deactivation activities, if the EOF was not activated 3.2 Emergency Response and Recovery Director 3.2.1 Exercise final authority for downgrading / deactivating emergency classification levels and declaring that a recovery phase is to p) be entered.

- Page 2 of 15 DEACTIVATION OF THE EMERGENCY RESPONSE ORGANIZATI0fl Revision 1 Date - 11/24/81 4.0 INSTRUCTIONS 4.1 Determination to Deactivate 4.1.1 Deactivation in part or in whole, may be considered when l conditions which constituted the emergency action level have been brought under control and no longer pose a threat to the health and safety of plant personnel or the public. The conditions are considered under control when the plant is in a stable state, necessary plant operating equipment is functioning properly and there is no potential for uncontrolled radiological releases.
4.1.2 The need for continued implementation of the EPIPs, particularly radiological monitoring and other related activities, may depend upon the residual conditions lef t by the emergency. Residual conditions may include such things as physical damage or radiological hazards and are corrected during the Recovery and Reentry Phase, but may require continued support from the Site -
Emergency Organization.
4.1.3 Attachment 2, "EAL De-escalation Guidelines" should be used as an aid in determining the plant parameters necessary to ensure and confirm a de-escalating condition.
- a. The major plant and met. alogical variables which must be determined and evaluated prior to de-escalating an EAL are divided into the following categories:
(1) Plant Operational 7arameters (2) Plant Radiological Parameters (3) Potential Radiological Release Parameters
- b. The attachment should be used as follows:
(1) Compile the information indicated at regular intervals (2) Evaluate the status and trend entries for critical systems which are directly or indirectly involved with the emergency events.
(3) Determine if de-escalation is warranted.

4.3.3 Each of the personnel identified as responsible for deactivation shall prepare a report to be sent to the Chief Engineer and other DAEC management, as appropriate. These reports ( Attachment 1) shall note the status and condition of personnel, f acilities, equipment and supplies and shall identify necessary further actions to be taken with each.
4.4 Restoration of Facilities, Equipment and Supplies As the Emergency Response Organization is deactivated, and as recovery operations are completed, the following organizations are responsible for restoring facilities, equipment and supplies to their original status during the recovery phase:

1.0 PURPOSE This procedure provides instructions for planning and performing post-accident recovery and reentry operations.
2.0 APPLICABILITY This procedure shall be implemented upon declaration of a recovery status at DAEC following an emergency condition. It is applicable to IELP and DAEC management and technical personnel involved in recovery operations.
NOTE Recovery represents that point following an emergency conditon when the plant is in a stable state, necessary plant operating equipment is functioning properly and there is no potential for uncontrolled radiological releases.
Attachment 2 "EAL De-escalation Guidelines" of EPIP 5.1," Deactivation of the Emergency Response Organization " can be used as an aid in determining the plant parameters necessary to ensure and confirm a de-escalating.c'ondition.
3.0 RESPONSIBILITIES 3.1 Emergency Response and Recovery Director 3.1.1 Coordinate and manage the recovery organization.
3.1.2 Ensure that sufficient personnel from IELP, OAEC, and outside organizations are available as necessary.
3.2 Emergency Coordinator 3.2.1 Coordinate and manage recovery activities at DAEC.
3.2.2 Ensure coordination between the plant staff and the corporate recovery organization.
3.2.3 Direct and coordinate the implementation of plans, procedures, and schedules to meet recovery operations.
3.3 Site Radiation Protection Coordinator 3.3.1 Establish and manage radiological controls for reentry and recovery activities.

~3 Date - 11/24/81 4.0 INSTRUCTIONS 4.1 Transition to Recovery 4.1.1 The Emergency Response and Recovery Director shall notify the Corporate Emergency Response Organization of the transition from emergency status to recovery status.
4.1.2 The Emergency Coordinator shall notify the Site Emergency Response Organization of the transition from emergency status to recovery status.
4.2 Radiological Prerequisites For Reentry 4.2.1 Prior to a planned entry into any area of the plant which has been subjected to physical or radiological damage, the following conditions shall be met:
- a. Establish objectives for reentry team ,
- b. Based on the extent, severity and location of the emergency and other plant data available, an optimum point of entry into the affected area shall be determined.
- c. The Emergency Coordinator shall grant authorization for entry.
- d. Exposure limits shall comply with RPP 2.1, " Personnel Exposure Limits"
- e. An entry team shall be selected as follows:
(1) One or more members of the team shall be a Ibalth Physics Technician.
(2) The team shall consist of a minimum of two persons.
(3) In order to minimize personnel exposure, the total nunber of team members and stay times shall be limited.
NOTE Activities of Health Physics personnel shall be limited to radiological monitoring functions.
\- f. Entry team personnel shall be briefed as described in Attachment 1.

NOTE Prior to entry, precautions shall be taken to ensure the safety of the entry team (s), including radiological and environmental assessment, surveys, atmosphere ambient temperature readings and gaseous or particulate surveys, if possible.
4.3 Reentry l 1
4.3.1 The entry team shall enter the affected area cautiously and shall perform the following tasks:
" Radioactive Contamination Surveys" and RPP 7.5 " Hazardous Atmosphere Surveys."
- b. Obtain visual data, or photographs, if possible. .
(]) c. Verify the operability of permanently installed I
communications within the entry area, time permitting.
4.3.2 Upon completion of initial observations and data collection, the entry team shall exit the affected area.
4.3.3 Decontanination activites shall be in compliance with RPP 12.1
" Personnel Decontanination," RPP 12.2 " Equipment and Materials l Decontamination," and RPP 12.3 " Area Decontamination."
4.4 Analysis of the Reentry Data 4.4.1 Personnel shall be debriefed as described in Attachment 2.
4.4.2 Determine if the objectives of the reentry were accomplished.
4.4.3 The operability of equipment as indicated in the Control Room shall be compared with the data taken during reentry as follows:
- a. Cross-check instrument readings of the radiation monitors against the survey readings.
- b. Compare the operable equipment with the non-operable equipment as recognized by the reentry team.

This procedure shall be implemented upon declaration of an emergency classified as an Alert, Site Emergency, or General Emergency. It is applicable to all site management and technical personnel assigned to the TSC and/or responsible for the emergency functions of the TSC.
I NOTE During an event classified as an Unusual Event, the Emergency Coordinator, at his discretion, may activate the TSC.
m 3.0 RESPONSIBILITIES O 3.1 Emergency Coordinator 3.1.1 Coordinate accident assessment and analyses efforts with the Radiological and EOF Manager to determine the potential or actual radiological impact of the emergency condition upon the public.
3.1.2 Coordinate efforts with the Emergency Support Manager, as necessary, to return the plant to a stable, safe condition.
3.1.3 Ensure performance of required actions by the Site Emergency Response Organization. See EPIP 2.3 " Emergency Coordinator Duties".

- g. Verify that notification of personnel has been initiated per EPIP 1.2, " Notification of Emergency Response Personnel and Offsite Support Agencies".
- h. Verify TSC manning status using Attachments 1 & 2 as guidance.

4.1.2 Security shall perform the following functions to activate the TSC:

- 1) The prime objective is to return the plant to a safe, stable configuration and terminate offsite releases.
- 2) Where several alternative courses of action exist, decisions as to which course to follow will be as directed by the Emergency Coordinator.
- b. Provide plant status information as requested by the EOF and NRC.
- 1) During the initial stages of the event the Parameter Status Board shall be updated every 15 minutes unless otherwise directed by the TSC Supervisor.
- 2) Information displayed on the status boards shall be recorded before being changed to serve as a historical record.
- 3) If the EOF is not manned, provide plant status and radiological information as requested to the Linn and Benton o County Emergency Operations Centers and the State of Iowa
() Office of Disaster Services.
- c. If radiological releases are in progress, assist the SSE in determining the source and the means available to terminate these releases.
- d. Where alternative courses of action taken to mitigate the consequences of the event are accomplished by placing systems in abnormal configurations, evaluate the effects such off-normal modes might have on future operational evolutions.
- e. As appropriate, develop emergency operating instructions for off-normal operating modes and insure that operating personnel are briefed upon relief or shift turnover.
- f. Assist the SSE with the identification of' temporary modifications or emergency repair work which, if accomplished, will assist in mitigating the consequences of the accident or terminating the releases.

- g. Perform neutron and/or thermal hydraulic analyses, as appropriate, using the computer facilities available to assist in determining the status of the core, ECCS System operating capabilities, etc.
- h. Identify expected changes in plant status based upon operational activities in progress or.which may be planned.
- 1) Should conditions become worse, escalate the classification of the emergency, if appropriate. ,
- 2) Should unexpected conditions occur, initiate actions to stop the escalation and reevaluate alternative corrective action steps.
- i. Perform research, as required, using as-built drawings, specifications and other engineering data to insure that technical evaluations are being conducted with the latest information and that operational evolutions are properly planned.
- j. Evaluate changes in plant status for potential radiological consequences to the public and apprise the EOF of all such changes and the results of the evaluations conducted.
4.2.2 The Security and Support Supervisor shall ensure the performance of the following functions:
- a. Assure that accountability checks are initiated and are progressing in an expeditious manner.
- b. Assure that the fire and evacuation alarms and related announcements made over the paging system are clear, concise and meaningful,
- c. Determine existing and projected administrative support needs and provide direction to the Administrative Supervisor.
- d. As directed by the TSC Supervisor define the requirements and provide direction to the Administrative Supervisor to provide long range personnel, material, facilities, food and other g) logistical support for the on site emergency response team.

- a. Coordinate the analytical and engineering efforts of site and l corporate engineering personnel.
