'Ihis is in response to R. C. Iewis' August 28, 1981 letter to H. G. Parris, Report Nos. 50-518/81-15, 50-519/81-15, 50-520/81-15, and 50-521/81-15, concerning certain activities at the Hartsville Nuclear Plants which have appeared to violate NBC requirements. his report responds to the Notice of Violation described in Appendix A of the OIE inspection report referenced above. %is is our final report on this item of noncompliance.
Noncomoliance Item - Severity Ievel V - Violation 520/81-15-01 10 CFR 50, Appendix B, Criterion V, as implemented by paragraph 17.lA.5 of the PSAR requires that activities affecting quality be prescribed by documented procedures and acconplished in accordance with these procedures. Paragraph 2 of Procedure CEP 5.04 requires that documents, quality control instructions, and other special instructions and/or procedures nacessary to perform the work be identified in the work pactage. Paragraph 3.0 of Procedure QCI N-504 requires that two preproduction stud welds be welded and tested prior to production stud welding.
Contrary to the above, on August 12, 1981, activities affecting quality were not accomplished in accordance with documented procedures in that work pad age B262-C2 for attachment of decking and studs to the control building structural steel did not reference the stud welding procedure or the stud welding inspection procedure and the two production test studs, for the stud welding in progress on the control bui! ding steel, had not been welded and tested.
This is a Severity Level V Violation (Supplement II.E).
Response
1.

W e Peasons for the violation his condition resulted. from oversight and failure to follow procedure. We rgineer responsible for initiation of the work package failed to identify the need to include a reference to the stud welding procedure or the stud welding inspection procedure. During review of the work package, Qui ~ity Control personnel also overlooked this requirement.
Procedures were not followed in that craft personnel proceeded with work not specifically addressed in the work package. QC inspections were also performed even though no inspection hold points were included in the package.

Corrective Steps Taken and Resulta kAleved_
Work package B262-C2 was revised on August 13, 1981, by the preparation of addendum No. 2 which adds stud welding procedures and stud welding inspection procedures.
All of the sted9 welded without qualification by testing of two production studs were bend tested. Since none fa!' 3 m.ese were considered acceptable.
4.
Corrective Steps Taken to Avoid Further Noin w eliance The responsible engineer is now aware of the requirements which need to be included for stud welding. As a result, all issued work packages involving installation of decking over structaral steel are being reviewed. If the stud welding procedures have been omitted, addenda will be initiated to add the stud welding criteria. In addition on September 10, 1981, QC inspectors were orally instructed to perform inspections in accordance with the work package instructions. Absence of instructions for the activity will be regarCed as a questionable condition requiring contact with the respc :sible engineer md the preparation of a QCIR if procedures have been violated.
Informal neetings were conducted on August 18 and September 1,1981, with engineering, inspection, and craft personnel in attendance. 'Ibese meetiys were held to assure that all involved are aware of procedural requirements, especially the requirements for stud welding.
