s APPENDIX A NOTICE OF VIOLATION Alabama Power Company Docket Nos. 50-348 & 50-364 Farley 1 & 2 License Nos. NPF-2 & NPF-8 As a result of the inspection conducted on June 16 - July 15,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following viclation was identified.
Technical Specification 3.0.4 requires that entry into an operational mode or other specified applicability condition shall not be made unless the conditions of the Limiting Condition of Operation are met without reliance on provisions contained in the action statements.
Contrary to the above, entry into an operational mode was made on two occasions without the required equipment being operable as specified in section 3.8.1.1 of the Technical Specification.
On March 17, 1981 Unit i reactor operational mode was changed from Mode 5 to Mode 4 while diesel generator 1-2A was inoperable. On May 29,1981, Unit 2 reactor operational mode was changed from mode 2 to mode 1 while generator 1-2A was inoperable.
This is a Severity Level IV Violation (Supplement I.D.2.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
