Dear Mr. Lewis:
This letter refers to the following violation in the subject inspec-tion report:
" Technical Specification 3.0.4 requires that entry into an opera-tional mode or other specified applicability condition shall not be made unless the conditions of the Limiting Condition of Operation are met without reliance on provisions contained in the action state-ments.
Contrary to the above, entry into an operational mode was made on two occasions without the required equipment being operable as specified in section 3.8.1.1 of the Technical Specification. On March 17, 1981 Unit 1 reactor operational mode was changed from Mode 5 to Mode 4 while diesel generator 1-2A was inoperable. On May 29, 1981, Unit 2 reactor operational mode was changed from Mode 2 to Mode 1 while generator 1-2A was inoperable."
Response
The following corrective action has been accomplished:
1.
The personnel involved have been counseled concerning the signi-ficance of this incident.

The Training Department has been notified to include these inci-dents in Licensed Operator requalification training.
3.
An Operations memorandum (81-08) has been written emphasizing proper shift turnover in order to insure that personnel know the the exact status of plant equipment.
