s WEEKLY INFORMATION REPORT DIVISION OF SECURITY WEEK ENDING APRIL 29, 1983 Central Personnel Clearance Index Computer System ADM/SEC and RM/D are entering into an agreement with the DOE Office of Safeguards and Security allowing SEC to use the DOE computerized Central Personnel Clearance Index (CPCI) for NRC personnel security clearance data.

Use of the DOE system will eliminate duplication of records and computer systems and will be both managerially and fiscally sound and cost-effective. The use of the DOE system should expedite security clearance processing of individuals of common interest (e.g., licensees, contractors, and federal employees / applicants) and reduce the paperwork burdens on individuals and their employers when requesting security clearance.

SEC's on-line capability to the DOE CPCI is expected to occur during September / October 1983.

KANAGEMENT DEVELOPMENT AND TRAINING STAFF WEEKLY IllFORMATION REPORT WEEK ENDING 4/29/83 SUPERVISORY DEVELOPMENT The first Group Orientation session of the new Pre-Supervisory Orientation Program (P-50P) was held on April 20-21, 1983. The P-SOP, which is a part of the overall NRC Supervisory Development Program, provides a means by which non-supervisory employees may explore their desire for career development toward a supervisory position. The Program consists of two parts: Part I - a self-study correspondence course entitled Applied Supervision, designed to clarify the major roles of the first-level supervisor; and Part II - a Group Orientation designed tA provide career guidance related to supervisory positions.

Currently, there are 365 NRC employees enrolled in the Program, both at Headquarters and tha Regional Offices. Additional Group Orientation sessions for employees who have completed Part I have been scheduled by MDTS for May 23 and 24 and June 16 and 17.

1 DIVISION OF ORGANIZI.fl0N AND PERSONNEL ITEMS OF INTEREST - APRIL 29, 1983 CHANGES IN THE REGIONAL VOLUNTEER PROGRAM On June 29, 1982, Regional Administrators were limited regarding outside hiring when NRC Headquarters employees are well qualified and available.

The same date, Headquarters employees were invited to register their interest in Regional positions with the Division of Organization and Personnel.

As of April 29, 1983, two more Headquarters employees had been selected by Regional Administrators.

This brings the total number of selections to 85, distributed by Region as follows:

The Task Force on Regional Generic Performance ElePents ar.d Standards met in Eethesda on April 19 and 20,1983, to cc.plete revising generic elements and standards for Inspector. positions in the Regions.

Report and recommendations were sent to Regional Administrators-by the Directcr, Office of ADM on April 26,'1983.

The dispute over the third article is being prieved by the Union; meanwhile the Management Negotiating Team has offered EDO-approved counterproposals and the parties are scheduled to banin negctiations no later than May 31, 19:3.

0FFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending April 29, 1983 Turkey Point Unit 3 On April 20, 1983, NRR was informed that manual isolation valves for the steam supply to the turbine driven auxiliary feedwater pumps were found closed on April 19.

Unit 3 was operating at power and Unit 4 is shutdown for steam generator repair.

The auxiliary feedwater system (AFWS) at Turkey Point consists of three steam driven pumps for the two units. The pumps are designated A, B, and C.

Pump A is out of service leaving pumps B and C for single unit operation.

Modifications were being implemented on i.he AFWS which include providing a second source of steam for the turbine driven pumps.

Tests, including hydros, were being performed on the new steam supply system from April 11 thru 13. On April 14, the isolation valves on the new steam supply systems were tagged closed.

The Turkey Point procedures require that the proper valve alignment for the AFUS be verified every four hours. On April 19 at 6:30 a.m. one manual isolation valve on each of the norrial steam supplies to the turbine driven pumps was discovered to be closed and tagged out.

The AFUS was inoperable.

The records indicate the valve alignment was verified every four hours; however, it cannot be determined at this time how long the AFWS was inoperable.

The licensee has notified the Ret.sent Inspector and the Region II Office and are taking the following actions as agreed to by the Region staff:

1.

Walkdown of the accessible portions of all safety related system flowpaths which verified that each accessible valve (manual, power operated and automatic) in the flowpath is in its correct position.

2.

Performed a review of all safety related clearances (in accessible areas) which ensured that they are correct and that the tags are.

properly placed.

3.

Items 1 and 2 above will be repeated once more approximately one week after the completion of the above items.

l 4.

Florida Power and Light Company personnel will meet with members of the NRC staff during the week of April 25, 1983 to discuss these issues.

Fort Calhoun Various areas within the auxiliary building and one area outside the l

auxiliary building were radiologically contaminated on the night of April 21, 1983.

ENCLOSURE B' APR 2 91983

A hose connected to the demineralized water supply was put in the refueling transfer canal. The nozzle to the hose was reported to be closed.

For some unknown reason, the nozzle opened and the canal filled and overflowed into the spent fuel pool.

(The axial height of the door between the refueling transfer canal and spent fuel pool is lower than the walls of the canal and spent fuel pool.) The spent fuel pool has an overflow drain that drains to theSafetyInjectionandRefuelingWater(SIRW) Tank. This is the path that the now contaminated water took. The SIRW Tank overflowed via the upper hatch, and the contaminated water made its way by gravity out of the auxiliary building via the bottom of the fuel shipment door.

Various areas within the auxiliary building are contaminated.

An area approximately 30 feet by 50 feet outside the auxiliary building is contaminated. The outside area is roped off.

Some surface areas are radioactively hot and measurements cre being taken.

The total quantity of liquid spilled is unknown at this time.

The boron concentration within the SIRW Tank and Spent Fuel Pool were reported to be within technical specifications. An unusual event was not declared by the plant management.

Maine Yankee - Loss of 115Kv Backup Offsite Power At approximately 1107 on April 25, 1983, Maine Yankee lost the one operable 115Kv incoming line. This line was its sole source of immediately available offsite power. The loss was caused by a lightning strike.

The line was restored.at 1452. The plant continued to operate at power during the outage.

Indian Point Unit 3 The Power Authority of the State of New York (PASfiY) presently estimates the restart of IP3 for mid to late May. The facility has been shutdown for more than a year in order to accomplish steam generator tube repairs.

Among other things, the licensee attributed steam generator tube failure to corrosion. As a result, by letter dated April 21, 1983, the staff requested the licensee's commitment to either the EPRI or Steam Generator Owner's Group PWR Secondary Water Chemistry Guidelines.

PAS!4Y has expressed a serious concern regarding the staff position.

Therefore, an Appeal Meeting has been scheduled for 9:00 a.m. on Friday, April 29, 1983 in Room P-114.

Arkansas fluclear One, Unit 1 (ANO-1)

Arkansas Power & Light Company (AP&L) has decided to remove the vessel head to inspect the upper core barrel bolting of ANO-1. This is the result of a concern identified in the ultrasonic (UT) inspection of the upper core barrel bolts of Rancho Seco and more recently Crystal River 3 (CR-3). At Rancho Seco, 19 bolts out of 120 had UT indications and at CR-3, 25 out of 65 APR 2 91983 ENCLOSURE B

. inspected had indications. The licensee estimates it will require approximately four weeks to complete the inspection and startup for power operation.

ANO-1 was shut down on November 8,1982 as a result of a reactor coolant pump seal leak. The unit then proceeded into a refueling outage.

During this outage the licensee perforned the 10-year inservice inspection of the vessel, the steam generator tube inspection, the electrical distribution verification test, the replacement of the turbine of the emergency feedwater pump and the reactor coolant pump fly wheel examination.

Thesteamgenerator(SG) tube inspection resulted in a 100% inspection of both SGs and the plugging of 45 defective tubes in "A" 0TSG and 86 defective tubes in "B" 0TSG.

Delays in 4

startup were due to the OTSG inspection and the operation of the EFW turbine.

During this time, ANO-1 experienced a control rod drive trip breaker failure.

Af ter reviewing information provided by the licensee, the staff prepared a safety evaluation.and authorized startup and operation. On April 6, ANO-1 went critical and in the operation of bringing the generator on line, a main turbir.e bearing failed.

The plant was returned to shutdown condition. ANO-1 was estimated to start up about April 28 when the licensee decided to inspect the upper core barrel bolts. The thermal shield bolts were inspected during the last outage.

Effluer.t and Environmental (Radiolooical) Information Liquid effluents from the TMI site released to the Susquehanna River, after sampling and monitoring, were within the regulatory limits and in accordance with NRC requirements and City of Lancaster Agreement.

During the period April 22, 1983, through April 28, 1983, the effluents contained no detectable radioactivity at the discharge point and individual effluent sources, which originated within Unit 2, contained minute amounts of radioactivity.

Calculations indicate that less than one ten-millionth (0.0000001) of a curie of cesium was discharged.

  • Uncertainties exist as to the exact location and accuracy of these readings.

This 10-cubic foot zeolite liner, which contained approximately 30,000 curies of mixed fission products (predominately Cesium-137, Strontium-90, and their daughter products) was loaded with a catalytic recombiner to maintain non-combustible conditions during the handling and shipment period.

0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending April 29, 1983 Futura Chemical / Cotter Corporation At the request of the Missouri State Department of Natural Resources (DNR) a meeting was held with Mr. E. Dean Jarboe (owner of Futura Chemical Company) to discuss the possibility of restricting access to a pile of soil contaminated with residues from the extraction of uranium from ore during World War II.

This soil is currently stored on Futura Chemical Company property in Hazelwood, Missouri.

Mr. Jarboe indicated that the contaminated soil was blocking an area needed to expand his business.

Rather than restrict the area by fencing, Mr. Jarboe proposed that Cotter Corporation, the former licensee, construct a building on the backend of his property large enough to store the material until a suitable disposal site is found.

DNR representatives agreed that Mr. Jarboe's proposal would satisfy the State's concerns.

West Valley Demonstration Project FCAF staff has received for review the information related to the Department of Energy's plans for designing and constructing a component test stand for the West Valley high level waste solidification project.

Contract Award On April 21, NUSAC, Incorporated, was awarded a contract under which they will be utilizing the augmented Automated Material Accounting Statistics System (AMASS) methodology to evaluate material accounting performance at various NRC - designated licensee fuel fabrication facilities.

NUSAC will be analyzing measurement control programs and measurement bias and error estimation proce-dures, as well as evaluating measurement and non-meansurement contributors to the uncertainty of the inventory difference.

The applicability of the AMASS methodology for analyzing process monitoring data also will be studied.

EPA Standard On December 29, 1982, the Environmental Protection Agency (EPA) published proposed environmental standards for management and disposal of high-level radioactive wastes.

The public comment period for the proposed standards closes May 2, 1983, and public hearings are scheduled for May 12, 13, and 14 in Washington, D.C. and May 19, 20, and 21 in Denver, C0.

EPA's Science Advisory Board has been reviewing the proposed standards since their publica-tion, and the next public meeting of the Board is scheduled for May 2 and 3 in Washington.

As a result, both standby gas treatment systems were inoperable for approximately one day in violation of a technical specification limiting condition for operation.

EN 83-26, Niagara Mohawk Power Corporation (Nine Mile Point - Unit 1) -

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $100,000 was issued April 26, 1983. This action is based upon the Licensee's failure to assure the adequate implementation of a quality assurance program by a principal contractor in that trainees who were not fully certified performed safety-related inspections and several inspection reports were signed by certified inspectors indicating they had performed an inspection, when in fact, the inspection had been performed by a trainee.

Since this violation involved the falsification

Emergency Preparedness FEMA made a presentation of their findings regarding offsite emergency preparedness implementation around the Indian Point facility to the Commission on April 20, 1983. The principal finding was that the implementa-tion was not sufficient to assure the health and safety of the public around the facility.

Foremost among those was the finding that the State of NY did not provide sufficient measures to compensate for Rockland Co. lack of participation. The staff is developing its recommendations to the Commission based upon the FEMA findins.

Temporary Instructions A Temporary Instruction is being prepared for regional folicwup to Bulletins 83-01 and 83-04.

In addition to providing additional guidance on review of testing maintenance, reporting of failures and safety cl'assification of components, the TI will request inspectors to identify and communicate to IE:HQ instances of previously unreported failures of W DB-Type breakers. A followup Bulletin is also being prepared to request identification and review of other applica-tions of undervoltage trip devices in safety-related systems.

Hayward Tyler Pump Company On April 20, 1983, members of the Engineering and Generic Communications Branch made a presentation to the CRGR on a proposed IE Bulletin on pumps and spare parts manufactured by the Hayward Tyler Pump Company. The CRGR approved issuance of the Bulletin, but sugar.sted some relatively minor revisions. These revisions are generally tuing incorporated into the Bulletin.

0FFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending April 29, 1983 Severe Fuel Damage Research Program Status Meeting i

A 2-day status meeting on the Severe Fuel Damage Research program was held with the foreign program partners and the program contractors in Idaho Falls on April 18 and 19. The results of the PBF 5evere Fuel Damage Scoping Test (SFD-ST) were reviewed, including the post-test neutron radiographs of the damaged fuel bundle and the tomographic reconstructions (like medical CAT scans) from these radiographs. The future program plans and status for th*e integrated research program were also reviewed.

In addition to the integral (multi-effect) tests in PBF, the program included integral check tests in NRU with full-length fuel bundles, separate-effects expetiments on debris fomation and relocation and on damaged-core coolability in ACRR, laboratory separate-effects experiments in the US and at KfK in the Federal Republic of Gemany, and development and verification of the SCDAP mechanistic severe-fuel damage code and the MELPROG mechanistic melt-progression code.

The foreign partners attending the meeting were the Federal Republic of Gemany, the United Kingdom, Japan. Italy, Belgium, and the Netherlands.

The review meeting was successful, and all of the foreign partners expressed strong support for the program. Each of the foreign partners contribute direct financial or substantial in-kind research to the program. The direct foreign financial support amounts to about 15% of the program cost, and this increases to about 25% when allowance is made for the in-kind research support.

The next SFD. program review among the partners will be held as a closed meeting in conjunction with the WPSR Infomation Meeting in October. The foreign partners expressed an interest in making this more of a two-way interchange of infomation, with 1 day for new results from the SFD program and 1 day for foreign partner results.

It was agreed that Dr. Ron Potter of the UK would coordinate the foreign partner participation.

i s PBF Severe Fuel Damage Test 1-1 The PBF Severe-Fuel Damage Test 1-1, scheduled for April 21 has been delayed by a small leak in the test train. This leak between the test fuel bundle and the coolant by-pass loop in the test train was detected at the start of the test transient following a 4-day operation at full power for fuel preconditioning and shutdown for build up of the proper cesium-iodine fission product ratio for the test.

l In-place tests to detemine the precise location of the leak are near completion.

It will then be detemined whether it is possible to run the test with an adjusted near-zero pressure drop across the leak for negligible leak flow, so that the test objectives will not be jeopardized.

If this is possible, the test will be perfomed in a week or two.

If not, the 1-1 test train will be removed from PBF and the 1-2 train installed, after making sure that the cause of the leak has been corrected in the 1-2 train. The 1-2 test would then be perfomed, ahead of schedule, in about a month, and the 1-1 test train, which is now radioactive, would be repaired so that the 1-1 test would be perfomed this summer. The decision on the course of action should be made this week.

They also discussed the May visit of Larry Shao, RES, to China to lecture on seismic design criteria and structural and mechanical design related to reactor vessels, steam generators and piping; the proposed one-year technical assignment of XUE Da-Zhi to BNL to work on NRC-sponsored research on probabilistic risk assessment; the development of China's nuclear power program; and continued NRC receipt of Chinese nuclear safety documents.

Intcrnational Agreements Within the past few weeks the staff has concluded agreements relating to cooperation in the NRC Severe Accident Research Program with Belgonucleaire and CEN/SCK-Belgium, ECN-Netherlands, ENEA-Italy and KfK-Federal Republic of Germany.

This final report incorporates the resolution of peer review comments from NRR, IE, RES, regional offices, INP0, and NSAC.

The report provides:

1) a brief summary of related actions for circuit breakers developed by the NRC; 2) results of review and evaluation of specific events involving failures of Class 1E circuit breakers to close on demand; and (3) suggestions and recommendations that, if implemented, could lead to improvement in the operational performance of these circuit breaker units. The results of the study suggest that the monitoring of the' closing circuit of such breakers is appropriate, augmented with additional measures relating to surveillance and maintenance procedures and practices.

In addition, the results suggest that training covering the associated logic and operation may be appropriate for breakers equipped with anti-pumping controls.

SECY 83-52A - Final Rulemakinc Concerning Licensed Operator Staffing at Nuclear ~ Power Units and Draft Policy Statement on Shif t Crew Qualifications.

The Commission by a vote of 5-0*,

approved a final rule (as attached) which requires (a) the presence of a person with a senior operator license ir the control 2

room at all times that a nuclear power anit is operating; and (b) provides a minimum number of appropriately licensed personnel on each shift at all times.

Commis-sioner Roberts, while approving the rule, disagreed with the modification made to the first paragraph of page 8.

'Section 201 of the Energy Reorganization Act, 42 USC Sec. .5841, provides that action of the Commission shall be determined by a " majority vote of_the members present."

"I have reservations about both the rule and the policy statement.

The following concerns should be addressed by the staff in its implementation of the staffing measures:

(1)

With regard to the policy statement, I feel that the BS degree requirement for the combined SR0/STA wil1 be difficult for licensees to maintain and, in some cases, will provide only limited improve-ments in the ability of an operating shift to handle unexpected transients or accidents.

The staff should maintain a flexible policy in applying the "or equivalent" criteria to take into account relevant training and experience of operators.

This flexibility will allow the licensee's to select those SR0/ STAS who have demonstrated the characteristics an~d analytical abilities intended by the Commission when it established the interim STA policy.

With regard to the staf fing rule, I am concerned that the SR0 control room limitation is too re s t ri c ti v'e.

I agree that the SR0 should remain current with plant conditions and be immediately-available to direct operator response; however, I am sure that infrequent and momentary excursions from the immediate control room vicinity could be accommodated with little or no added public risk.

Flexibility in the implementation of this restric-tion is appropriate.

I am-also concerned over the prospects of further-

. changes to plant ' staffing requirements as-a result of the task analysis studies.

The staff should keep the Commission informed of the study results and should encourage industry and public participation in the development of any proposed changes."
