EVENT DESCRIPTION

With the unit at 100% power, a routine walkdown of the control board revealed the circuit breaker for MOV-2867B (Beron Injection Tank Inlet) to be oper..

These breakers had been verified closed 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> earlier while performing the control board indicating light walk-down. The redundan flow path was checked to ensure it was energized.

This event is contrary to T.S.3.3.A.7 and reportable in accordance with T.S.6.6. 2.b (2).

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT EQUIPME'iT:

Upon receipt of a safety injection, signal, the BIT valves are automatically opened and the BIT contents are pumped into the RCS by the High Head Safety Injection Pumps. The redundant BIT flow path, MOV-2867A, was operable through-out this event; therefore, the health and safety of the public were not affected.

CAUSE

The breaker is located in an area where extensive design change activity was unuerway.

It is likely that the breaker was accidentally bumped, causing it to open.

4.

IMMEDIATE CORRECTIVE ACTION

The breaker was closed and the valve responded properly.

5.

SUBSEQUENT CORRECTIVE ACTION:

None required.

6.

ACTIONS TAKEN TO PREVENT RECURRENCE:

The number of construction personnel authorized access to the switchgear room has been reduced.

7.
