ERROR IN FLUSHING PT 1.

DESCRIPTION OF EVENT

A station review of the adequacy of the Feriodic Test program disclosed that the ' sensitized pipe supplied by containment spray pump 2-CS-P-1 A was being (flushed for less than the minimum time required by T.S. table 4.1-3B.

Periodic Test 17.1 called for a 15 minute flush, while the Technical Specification requi res a 20 minute flush. This incorrect procedure was used for the monthly flushes from Septmenber 1980 to July 1981. This event is contrary to T.S.4.1.E and reportable per T.S. 6.6.2.b.(3).

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS:

The flushing duration requirements of Table 4.1-3B ensure that a volume of water greater than that contained in the stagnant flow paths will be flushed in order to ensure that buildup of contaminants will not occur, thus reducing the potential for stress corrosion cracking.

In addition, the Technical Specifi-cation requires sampling of the flushed lines to demonstrate the effectiveness of the flushing in removing contaminants.

A review of the Periodic Test results for the months in which the short flushes werc performed has verified that the post-flush fluoride and chloride concentrations were within the acceptance criteria, indicating that the flushes were effective.

In addition, the redundant containment spray piping was flushed correctly.

Therefore, the cor.tainment spray system would still have performed its intended function following an accident, and the health and safety of the public were not affected.

CAUSE

The incorrect flush time was due to an inadvertent change to the Periodic Test flush time for pump 2-CS-P-1A piping. The change to the test procedure had on'y been intended to change the flush time for pump 2-CS-P-1B piping.

IMMEDIATE CORRECTIVE ACTION

A request:,to change Periodic Test 17.1 was submitted.

SUBSEQUENT CORRECTIVE ACTION:

The Periodic *.est was corrected.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

The flushing Periodic Tests were reviewed to ensure that all other flush times were in accordance with the Technical Specifications.
