Individual Rod Position Indication - 2D2 - Inoperable.

This report was initiated by Incident Report 81-308 CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 75% - Unit Load 750 MWe DESCRIPTION OF OCCURRENCE:

On August 7, 1981, the control operator noticed that'the individual ~

rod position indicator and control rod bank demand indicator for.

rod 2D2 were in disagreement by greater than + 12 steps.

The individual rod position indicator was declared inoperable at 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br /> and action statement 3.1.3.2.1 was entered.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

f DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure. The individual rod position indicator (IRPI) for control rod 2D2 was not in agreement with the control rod bank demand i

position due to drift in the calibration of the signal conditioning module.

j ANALYSIS OF OCCURRENg:

Technical Specification 3.1.3.2.1 requires that in modes 1 and 2 with a maximum of one rod position indicator per bank inoperable either:

Determine the position of the non-indicating rod (s) indirectly by the movable incore detectors at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod's position, or 2.
