IDENTIFICATION OF OCCURRENCE:

Missed Surveillance - Channel Check of Post Accident Monitoring Instrumentati c.. (SP (0) 4. 3. 3. 7)

CONDITIONS PRIOR TO OCCURRENCE:

Various Operational Modes DESCRIPTION OF OCCURRENCE:

During the review of surveillance procedures in preparation for mode change it was determined that the surveillance for Technical Specifi-cation 3.3.3.7 was overdue.

The surveillance is required to be performed every 31 days and was last perforced April 22, 1981.

Even though a large majority of the Post Accident Monitoring Instrumentation is channel checked every 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> during routine control console readings the surveillance of 3.3.3.7 was not completed in its entirety.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

l Personnel error.

A new surveillance procedure had been issued to meet the surveillance requirements of Technical Specification 3.3.3.7.

l Previously these requirements were met thought channel checks made when the control console reading were taken once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

When the procedure was issued personnel failed to enter it into the Inspec-tion Order System.

Therefore, when the surveillance became due no j

notification was made through the Inspection Order System to complete the surveillance.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.7.b requires that with the number of l

operable accident monitoring channels less than the minimum channels required, restore the inoperable channels to operable status within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> or be in at least hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION

When it was determined the surveillar.;e was overdue, the surveillance was completed immediately.

The surveillance procedure was entered into the Inspection Order System and the personnel responsible were counseled on the importance of ensuring new or revised surveillance requirements were entered into the Inspection Order System for tracking.
