i Identification of Occurrence:

Crystal River Units 1 & 2 startup transformer was inoperable contrary to Technical Specification 3.8.1.1.

Conditions Prior to Occurrence:

Mode 1 power operation (100%).

Description of Occurrence:

At 1055 during normal operation, Crystal River Units 1 & 2 startup transformer was rendered inoperable during maintenance on the 4160 V bus supplying the Units 1 & 2 house services transformer. Maintenance was initiated and operability was restored at 0635 on July 1, 1981.

Designation of Apparent Cause:

The cause of this event is attributed to personnel error.

Con-tractor maintenance personnel at the Crystal River fossil plant shorted the 4160 V bus supplying the startup transformer when they erroneously checked the energized section of the 4160 V bus supplying the house services transformer.

Analysis of Occurrence:

Redundancy was provided by two Emergency Diesel Generators and one independent offsite power supply. There was no effect upon the health or safety of the general public.

Corrective Action

The fotsil plant is providing covers for the startup bus sections that were involved, and they will post signs to pro-vide proper warning and instructions for future maintenance activities.

Failure Data:

This was the first occurrence of this type and this is the twenty-fifth event reported under this Specification.
