June 16, 1981 Identification of Occurrence:

Loss of all AC power contrary to Technical Specification 3.8.1.1; and Reactor Building temperature greater than 130*F, contrary to Technical Specification 3.6.1.5.

Conditons Prior to Occurrence:

Mode 1 power operation.

Description of Occurrence:

On June 16, 1981 at 2340 during Mode 1, lightning struck the 230 KV feeder line. The Lightning Arrestor System failed to prevent loss of the CR-3 startup transformer, resulting in loss of all AC power. Due to loss of AC power, air handling fans tripped causing Reactor Building temperature to be greater than 130*F for fifteen minutes on June 17, 1981.

Maximum Reactor Building temperature reached was 135'F.

Upon loss of AC power, Emergency Diesel Generator "A" was started to load "A" 4160 Volt Engineered Safeguards Bus; and "B" 4160 Volt engineered safeguards Bus was repowered by switching power to CR-3 4160 Volt engineered safeguards Bus "B" from the fossil plant startup transformer. Air handling fans 1B and 1C were restarted, and Reactor Building temperature was less than 130*1 by 0025 on June.17, 1981. Power was restored into the plan't through the CR-3 startup transformer on June 17, 1981 at 0436.

Designation of Apparent Cause:

The cause of these events is attributed to failure of the Lightning Arres-tor System to prevent loss of the CR-3 startup transformer, resulting in loss of all AC power. Loss of AC power caused the air handling fans to trip, resulting in high Reactor Building temperature.

Analysis of Occurrence:

Redundancy was provided by one Emergency Diesel Generator and one inde-pendent offsite power supply.

There was no effect upon the health or saf 'y of the general public.

Corrective Action

Corrective actions during the unit blackout were in accordance with Procedure EP-101, " Unit Blackout". An engineering investigation of the Lightning Arrestor System failure will be concluded by 31 January 1982.

Once high Reactor Building temperature was determined, the air handling fans were restarted and temperature returned to less than 130*F. No further corrective action is deemed necessary.

These are: the twenty-fourth event to be reported under Technical Specification 3.8.1.1; and the sixth event reported under Technical Specification 3.6.1.5.
