DENTIFICATION OF OCCURRENCE

Between June 4 and June 9, 1981, eight hydraulic shock suppressors (snubbers) were found to be out of oil and therefore, inoperable.

Since reactor operation had occurred during the surveillance interval, this is reportable as a degraded mode of LCO 4.3.10 per Fort St. Vrain Technical Specification AC 7.5.2(b)2.

EVENT DESCRIPTION:

The following is a table of events for the situation:

Date Event June 4, 1981 Snubbers HRS-104, HRS-357, and MSS-118 found to be out of oil and inoperable.

June 5, 1981 Snubbers BFS-142 and BFS-405E found out of oil and inoperable.

i June 9, 1981 Snubbers BFS-142E, BFS-532, and BFS-556 found out I of oil and inoperable.

l l CAUSE DESCRIPTION:

! The loss of oil from the snubber reservoirs was found to be due to leaky seals and 0-rings.

The corrective actions for the eight snubbers is as follows:

BFS-142 - Replaced reservoir gaskets, tested, and filled with oil.

BFS-532 - Cleaned, replaced seals and 0-rings, tested, and filled with oil.

BFS-556 - Cleaned and replaced seals, 0-rings, and gaskets. Filled with oil.

BFS-142E - Cleaned and replaced seals and 0-rings. Filled with oil.

BFS-405E - Replaced reservoir seal gaskets and filled with oil.

HRS-104 - Replaced 0-rings anc filled with oil.

HRS-357 - Filled with oil.

MSS-118 - Cleaned and replaced seals and 0-rings. Filled with oil.

Because eight snubbars were found inoperable, the surveillance interval was adjusted to 31 days per SR 5.3.8(a).

No further corrective action is anticipated or required for the eight snubbers described in this report.
