Check Valves 21MS46 and 23MS46 - Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

During the repair of a valve bonnet leak on check valve 23MS46 the valve was found to be damaged internally.

The valve disc stud had broken and the valve disc was in the bottom of the valve body.

Valve 23MS46 also had cracks in the disc, cracked bushings, warped hinge pin, and all hinge pin holes were elongated.

Valve 21MS46 was inspected and similar damage was observed.

This occurrence constituted failure of one or more components which could prevent the fulfillment of the functional requirements of

stems used to cope with accidents analyzed in the SAR in accordance wich Technical Specification 6.9.1.8.e.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this damage has not been determined and is under investigation.

ANALYSIS OF OCCURRENCE:

The vendor was called in to assist PSE&G personnel in determining the cause of this occurrence.

The representive supplied parts to return the valves to operable status.

CORRECTIVE ACTION

Valves were repaired and made operable.

Investigation of this i

l occurrence will determine the corrective action necessary to mitigate l

l a reoccurrence of this failure.

These valves will be radiographed until a determination of the reason for the damage can be made.

When this investigation is complete a supplemental report will be l

issued.
