No. 21 Auxiliary Feedwater Pump - Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 9% - Unit Load O MWe DESCRIPTION OF OCCURRENCE:

During start-up operations on May 30, 1981, No. 21 Auxiliary Feed Pump outboard bearing went into high temperature alarm and was removed from service.

Investigation of the alarm, found the out-board bearing and seal assembly had failed.

At 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br /> the Auxiliary Feed Pump was declared inoperable and Action Statement 3.7.1.2.a was entered.

Specification 6.9.1.9.b.

l DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

No. 21 Auxiliary Feed Pump outboard bearing and seal failed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.1.2.a requires that with one auxiliary feed pump inoperable, restore the required auxiliary feed pumps to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in hot shutdown within the following 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.

CORRECTIVE ACTION

The radial bearing, thrust bearing, mechanical seal and rotating assembly were replaced.

The No. 21 Auxiliary Feed Pump was tested satisfactorily and returned to operable status.

The Action Statement was terminated at 0512 hours0.00593 days <br />0.142 hours <br />8.465608e-4 weeks <br />1.94816e-4 months <br /> on June 2, 1981.
