MCGUIRE NUCIL\R STATION INCIDENT REPORT Report Number: R0-369/81-12 Report Date: February 20, 1931 Occurrence Date: February 5, 19Si Facility: McGuire L* nit 1, Cornelius, North Carolina Identification of Occurrence: Isolation Valve NV-250 was not Verified Closed Administratively Every 72 Haurs Following Completion of Initial Fuel Loading Conditions Prior to Occurrence: Initial Fuel Loading, Mode 6 Description of Occurrence:

On February 11, 1981, isolation valve NV-250 was found not to have been verified closed under administrative control as required by Technical Specification 4.9.1.3.

Apparent Cause of Occurrence:

Daily Surveillance Items did not contain a sign-off step to verify that NV-250 was closed.

Analysis of Occurrence:

Initial Fuel Loading test procedure required that NY-250 be verified closed administratively per Technical Specification 4.9.1.3. NV-250 was last verified locked closed per the above TP at 1825 hours0.0211 days <br />0.507 hours <br />0.00302 weeks <br />6.944125e-4 months <br /> on February 2, 19S1. The status of NV-250 was not checked again until 1955 hours0.0226 days <br />0.543 hours <br />0.00323 weeks <br />7.438775e-4 months <br /> on February 11, 1981, and was found to be locked closed with the White Equipment Danger Tag attached. Operations misinterpreted Technical Specification 4.9.1.3 to mean that NV-250 should be verified closed only when initial fuel loading was in progress. This was not the case, and thus was reportable.

Since NV-250 was locked closed when it was checked on February 11, 1981 it can be assumed that it had been closed since FrTruary 2, 1981. However, even if NV-250 had been open, there were other valves downstream of NV-250 which were locked closed or normally closed which would have prevented a boron dilutien of the Reactor Coolant System. Thus, the incident did not affect safe plant operation and_the health and safety of the public were not affected.

Corrective Action:

A procedure change was-incorporated into Daily Surveillance Items to verify that NV-250 is verified locked closed under administrative control.
