February 4,1981 Identification of Occurrence:

Auxiliary Building Ventilation Exhaust System Filter System Methyl Iodide removal efficiency <95% contrary to Technical Specification 3.7.8.1.

Conditions Prior to Occurrence:

Mode 1 power operation (98%).

Description of Occurrence:

While reviewing charcoal test results, it was discovered that filter system AHFL-2D methyl iodide removal efficiency was 92.14%. Maintenance had been previously initiated and the filter cells were replaced with other cells containing new impregnated charcoal. A functional test was performed satis-factorily and operability was restored at 1500 on February 5,1981.

Designation of Apparent Cause:

A number of contributing factors led to the deterioration of the charcoal filters. Pollutants in the atmosphere such as flue gases, Iodine adsorption, and normal air flow cause a loss of charcoal impregnant efficiency.

Anslysis of Occurrence:

There was no effect upon the health or safety of the general publice Due to the large number of factors involved in the deterioration of the charcoal and becuase desorption of various contaminants may cause erratic filter efficiency, there is no quantitative method for trending efficiency to determine when a bank will drop below the required efficiency.

Corrective Action

Monitoring of the charcoal filter banks will comply with Tech-nical Specification surveillance requirements. No further cor-rective action deemed necessary at this time.

Failure Data:

This was the second occurrence of inadequate fiiter efficiency and this is the twelfth event reported under this Specification.
