Team Inspection

The NRC staff conducted an Augmented Inspection Team (AIT) Inspection at the

'

Oyster Creek Nuclear Generating Station in Forked River, New Jersey..on

December 8 through 12, 1989. The purpose of this inspection was.to review the

.

'

facts and circumstances surrounding an event involving a main condenser low

vacuum transient without scram, either automatically or manually. We discussed

,

the results of that inspection with you and your staff at a management meeting

on December 12, 1989, and addressed the team's findings and conclusions.in the

enclosed report.

The team summarized its major findings and conclusions in section 1.4 of the

'

'

enclosed report. With respect to plant performance, the reactor protection

system (RPS) low vacuum trip functions for the "A" and "B" condensers were not

!

operable consistent with Technical Specifications (TS) in that their setpoint

had drifted below the TS minimum limits.

However, the team concluded that

these trip functions would have actuated (at the lower setpoint) to trip the

l.

plant.

With respect'to operator performance, the team determined that operator error

'

caused the event and their mitigating actions were prompt.

Further, at the

'

time of the event, the operators had conflicting information that a scram

.

condition existed and that there was a failure to scram. Overall, considering

,.

-the short' time period of the event, the team concluded that the operators'

actions were adequate.

The team further found that one indication channel did

'

indicate a vacuum below the scram setpoint but it was initially discocnted

until another verifiable indication could be received.

With respect to management performance, actions were adequate in responding

',

to the event and in cooperating with our inspection team. However, we note

that certain of the team findings were not similarly identified in your

- review or in past reviews of licensed activities, specifically, the technical

l

specification adequacy and instrumentation setpoint issues.

We would like to discuss these findings and conclusions with you at a manage-

ment meeting in the Region I office on a mutually agreeable date. At that

meeting, you should be prepared to discuss the issues summarized in section 1.4

of the report, any related actions taken or planned, your perspective on these

findings including any points of disagreement or errors in facts.

You should

also confirm or re-define your short and long term actions as listed in this

report (See Section 5.2 of the report). We will contact you shortly to confirm

a date for this meeting.

In accordance with 10 CFR 2.790 of the Commission's regulations a copy.of this

letter and its enclosure will be placed in the NRC Public Document Room.
