Valve 2PR6 Open/ Closed Indication Malfunction CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

Action Statement 3.4.5.b was entered at 1551 hours0.018 days <br />0.431 hours <br />0.00256 weeks <br />5.901555e-4 months <br /> on May 23, 1981, when valve position indication could not be obtained in either the open or closed position.

The valve was declared inoperable, closed, cnd power to the motor operator removed.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

M chanical failure.

2PR6 would not indicate either open or closed due to the motor operator limit switch being out of adjustment.

ANALYSIS OF OCCURRENCE:
