3 SUPPLEMENTARY INFORMATION j Report No.: 50-302/81-029/03L-0 Facility: Crystal River Unit 3 Report Date: June 23, 1981 Occurrence Date: May 22, 1981 Id:ntification of Occurrence: Emergency Feedwater Ultrasonic Flowrate Indicator was inoperable contrary to Technical Specification 3.7.1.2. Conditions Prior to Occurrence: Mode 1 power operation (100%). De:cription of Occurrence: At 0705 during normal opcration, it was discovered that FW-313-FT, Emergency Feedwater Ultrasonic Flow Indicator was in the fault condition and would not reset. Maintenance was initiated,and opera-bility was restored at 1345. Designation of Apparent Cause: The cause of this event is attributed to a failed power supply board in the flow transmitter. An01ysis of Occurrence: There was no effect upon the health or safety of the general public.

Corrective Action

The board was replaced. Long term corrective action will be to replace the ultrasonic flow transmitter with a conventional flow transmitter. This will be accomplished concurrently with major Emergency Feedwater System modifications presently scheduled for 1983. Failure Data: This was the first occurrence of this type,and this is the ninth event reported under this Specification. /rc }}
