Subject: NRC Investigation of Allegations at Washington Nuclear Projects 3 and 5

This refers to the inves:igation cencucted by Messrs. Bishop, Haist, Shackleton,

and Joukoff of this offt e on Septenher 25 to October 30, 1980 of activities

authorizec by NRC Con %ruction Permit No. CPPR-154 and 155, and to the discussion

of our finding:, hdo by Mr. Bisnop with Mr. J. C. Lockhart and other memcers

of your staff at the conclusion of the inspection.

Tha investigation was initiated of the heating, ventilation and air conditioning

(HA} contractor's work at WNP 3/5 because of allegations made regarding

withholding of records from NRC inspection and veroal instructions to falsify

cuality records. The investigation was subsequently expanded to other areas.

Within the areas, the investigaticn consisted of selective examinations of

precedures and representative records, interviews with personnel, and, observations

by the inspector.

Based on the results of this investigation it appears that one of your activities

was not conducted in full compliance with NRC requirements, as set forth

in the Motice of Violation, enclosed herewith as Appendix A.

This item of

noncompliance has been categorized into a level as described in Federal Register

Notice (45 FR 66754) dated October 7,1980.

Cur findings demonstrate the continuing need for WPPSS to improve the quality

assurance perfor ance of their contractors. Particularly those engaged in

activities affecting the quality of equipment important to safe operation

of the facility. We believe it is important that the lessons learned during

reviews of contractor activities at the WNP-2 project be applied to each of

the WPPSS nuclear projects.

In concert with Mr. Stello's letter to you dated

June 17, 1980, requesting additional infonnation regarding quality assurance,

we are concerned that the lessons learned in controlling contractor activities

are not being effectivel! performed at other WPPSS nuclear projects. Therefore,

in your response to the enclosed item of noncompliance, you are requested

to include a discussion of those actions being taken at WNP 3/5 to implement _

the lessons learned at WNP-2 to improve the contractor perforrance. The discussion

should also include a description of those corrective actions taken or planned

to assure that, in the future, a ccmolete file of all quality assurance records

generated will be maintained and available for inscection at all times and that

all such records will accurately reflect the status of the activities covered.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and the enclosed

inspection report will be placed in tre NRC's Public L mnt Room.

If this

report contains any information that you believe to be proprietary, it is

necessary that you submit a written application to this office, within 30

days of the date of this letter, requesting that such information be withheld

from public disclosure. The application must include a full statement of the

reascns wny it is claimed that the information is proprietary. The application

should be prepared so tnat any proprietary infonnation identified is contained

in an encicsure to the application, since the application without the enclosure

will also be placed in the Public Cocument Room.

If we do not hear from

you in this regard within the specified period, the report will be placed

in the Public Document Room.

to discuss them with you.
