i SUPPLEMENTARY INFORMATION

~

Report No.:

50-302/81-016/03L-0 Facility:

Crystal River Unit 3 Report Date:

March 31,1981 Occurrence Date:

March 4,1981.

Identification of Occurrence:

One (1) independent ECCS Subsystem inoperable contrary to Technical Specificatidn 3.5.2.

Conditions Prior to Occurrence:

Mode 5 cold shutdown.

Description of Occurrence:

At 0440 during performance of SP-340 (ECCS Pump Operability), the breaker for DHV-110, "A" Decay Heat Pump Discharge Throttle Valve, tripped while DHV-110 was being cycled from the full open to the closed position. Decay Heat Loop "B" provided redundancy. Main-tenance actions were initiated and DHV-110 was declared operable at 0640 after repetitive successful cycling.

Desicnation of Apparent Cause:

The cause of this event is unknown.

Analysis of Occurrence:

There was no effect upon the plant or general public.

Corrective Action

Electrical checks of the motor and control circuits failed to reveal the cause of the breaker trip. Satisfactory amp readings were realized during two (2) subsequent valve cycles. At 0640, DHV-110 was repetitively cycled with no isilures and has functioned l

properly to date. No further corrective action is deemed necessary at this time.

Failure Data:

This is the first occurrence of this type for DHV-110, and the fourteenth event reported under this Specification.
