January 26, 1981 Identification of Occurrence:

Failure of RM-A5 to start contrary to Technical Specification 3.7.7.1.

Conditions Prior to Occurrence:

Mode 1 power operation. (99%)

Description of Occurrence:

At 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> after routine filter change Radiation Monitor RM-A5 would not restart.

Control Complex ventilation was placed in Emergency Mode full recirculation and maintenance was initiated. Operability was restored at 1400.

Designation of Apparent Cause:

The cause of this event is attributed to a blown fuse.

Analysis of Occurrence:

there' was no effect upon the hea'.th or safety of the general public.

Corrective Action

No fault was discovered in the circuit. The fuse was replaced and operability was restored. No further corrective action deemed necessary.

Failure Data:

This was the first occurrence of this type, the fourth oc-l currence for RM-A5 and -the sixth report under thi, Specifica-tion.
