Final IDENTIFICATION OF OCCURRENCE:

During the annual calibration of the primary coolant dewpoint moisture moni-tor flow instrumentation, it was discovered that five of the sample flow circuits were out of calibration either before or af ter the surveillance procedure was completed. This is reportable per Fort St. Vrain Technical Specification AC 7.5.2(b)1 as setpoints being less conservative than required and per Fort St. Vrain Technical Specification AC 7.5.2(b)2 as a degraded mode of LCO 4.4.1, Table 4.4-1.

EVENT

DESCRIPTION

While performing the annual calibration of the primary coolant moisture scram instruments, plant instrument personnel observed that five of the sam-ple flow instruments were out of calibration in the non-conservative direc-tion and could have resulted in the operation of the primary coolant moi-sture scram instruments with the sample flow lower than required by note (c) of LCO 4.4.1.

The instruments were recalibrated, as required, and the sur-veillance completed.

Review of the surveillance data at a later date revealed that five of the -

4 sample flow instruments were left out of calibration in the non-conserva-tive direction. The instruments could not be brought into calibration be-cause of shift of the characteristic curve. The Results Engineering Super-visor analyzed the data and provided new calibration data for the sample flow instruments to insure that the sample flows are conservative with re-spect to the limits of LCO 4.4.1 note (t).

The new calibration data was also incorporated into the sample flow alarm functional test to provide the proper alarm setpoints to ensure compliance with the sample flows alarm setpoints specified in note (t) of LCO 4.4.1.

The monitors vere still capable of providing a trip input to the plant pro-tactive system-on high moisture. The moisture monitor system is operated with sample flows -in excess of the minimum required.

CAUSE

DESCRIPTION:

The cause of the instruments being out of calibration was the changs of

- characteristics of the flow elements during the time the elements have been in service. The individual changes in the flow elements characteristic curves has necessitated the use of individual calibration curve and alarm setpoint values for each of the separate flow elements.

CORRECTIVE ACTION:

The revised calibration values for each of the flow elements is being incor-porated into the annual calibration procedure and the monthly alarm functional test.

The sample flow alarm setpoints were adjusted to the new calibration values to assure that the alarm requirements of LCO 4.4.1 note (t) are met.

No further corrective action is anticipated or required for the calibration change of the FE's as stated in this report.
