January 23, 1981 Identification of Occurrence:

Open breaker for Control Complex Emergency Ventilation Fan AHF-18A contrary to Technical S?ecification 3.7.7.1.

Conditions Prior to Occurrence:

Mode 1 power operation (73%)

Description of Occurrence:

At 1710 during normal operation it was discovered that the indication liaht for Control Complex Emergency Ventilation Fan AHF-18A was off.

Investigation revealed that the breM cr was open fo. no reason. The breaker was shut and operabiiity was restored at 1804.

Designation of Apparent Cause:

The cause of this event is attributed to inadvertent bumping

'of the breaker.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action

All personnel have been advised to exercise extreme caution when workiag around electrical breakers.

Failure Data:

This is the first occurrence for AHF-18A and this is the fifth event reported under.nis Specification.
