Crystal River Unit 3 Report Dates 6 February 1981 Occurrence Date:

January 9, 1981 Identification of Occurrence:

Failure to have a containment isolation valve operable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 1 power operation (97%).

Description of Occurrence:

At 2245 af ter sampling the pressurizer steam space, it was discovered that containment isolation valve CAV-1 would not close. Manual valves CAV-133, 323, 330 and 336 were closed and maintenance was initiated. Opera-bility was restored at 0600 on January 10, 1981.

Designation of Apparent Cause:

The cause of this event is attributed to failed contacts in the torque switch of the valve operator.

Analysis of occurrence:

There were no effects upon the health or safety of the general public.

Corrective Action

The torque switch was replaced and the valve was satis-factorily tested. No further corrective action is deemed necessary at this time. A review of failure of Contain-s{ e E e
