Identification of Occurrence:

Failure to use approved seal material in snubbers contrary to Technical Specification 3.7.9.1.

Conditions Prior to Occurrence:

Mode 1 p<m 3roperation(98%).

Description of Occurrence:

At 0830 during normal operation it was discovered that approximately thirteen (13) snubbers had 0-rings installed that were not fabricated from ethylene propylene or other approved material.

Designation of Apparent Cause:

The cause of this event is attributed to failure to identify nonconforming materials in the receiving inspection.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public. The snubbers were determined to have Buna-N 0-rings installed. The material in the 0-rings was evaluated to be compatible with the operating environment.

Corrective Action

A letter requesting approval of the Buna-N seal material for use until the fall 1981 refuelling outage was sent to the USNRC. Approval was granted; operability was restored at 1700 on December 19, 1980. The seals will be replaced during the next refuelling outage. Quality Operating Procedures have been revised to improve control. Personnel have received training.

Failure Data:

This was the first occurrence of non-approved seal material and the third event reported under this Specification.
