December 8, 1980 Identification of Occurrence:

Failure to have containment isolation valves operable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 3 (hot standby 0%)

Description of Occurrence:

At 0950 on December 8,1980 after sampling the pressurizer water space, it was discovered that containment isolation valve CAV-3 would not close. Manual isolation valve CAV-128 was closed and maintenance was initiated. Operability was restored at 1230 December 8, 1980. At 2210 on December 9, 1980 CAV-3 again failed to close.

CAV-2 was closed and deactivated. Maintenance was initiated. Operability was restored at 2118 on December 11, 1980.

Designation of Apparent Cause:

The cause of the first event was attributed to inadequate clos-ing torque. The cause of the second ev?nt was attributed to a faulty torque switch.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action

i The valve stem for CAV-3 was lubricated after the first event and the functional test was satisfactory. After the second event the torque switch was replaced and the valve was satis-factorily tested. No further corrective action required.

Failure Data:

These are the fourth and fifth occurrences for CAV-3 and this is the thirtieth event reported under this specification.
