Attached is South Carolina Electric and Gas Company's response to the reference report. which summarizes Mr. J.L. Skolds' inspection July 21, 1980 through August 31, 1980. Specifically, we have addre:=ed the infraction dealing with improperly performing a system valve lineup during Hot Functional Testing. We consider that the actions we have taken are appropriate to assure that this type of event will not recur.

Summer Nuclear Station, Unit fo. 1 Docket No.: 50-395 Report No.: RII:JLS 80-25/ Notice of Violation Event Date: August 14, 1980 Description of Event On August 14, 1980, while performing a valve lineup as part of Preoperational Test Procedure RC-1, " Hot Functional Testing", valves 8353A, B, and C, 8357A, B, and C were initialed as "0 PEN", when in fact these valves do not exist.

Also valves 8540A, B, and C were initialed as not applicable ("N/A"), when in fact these valves exist and were closed.

Probable Consequences There are no consequences from these events, because the Reactor Coolant Pumps' Seal Leakoff Flows and the Standpipe Lo Level Alarms were in operation.

Cause of Event The valve lineup that was being used during the conduct of RC-1, " Hot Functional Testing", was prepared on January 25, 1979, and did not reflect current as built conditions. Valves 8353A, B, C, and 8357A, B, C had been initially installed to isolate Reactor Coolant Pump Seal Leakoff Flow Transmitter FT156B, Fr155B, and FT154B as applicable. A change to the system deleted these valves, but the change was not reflected in the procedure Valve Lineup. The operator recognized the purpose of these valves from the descriptions given in the procedure, observed that he had indicated Seal Leakoff Flow, and therefore assumed that in order to have Flow Indication, the valves must be open. Valves 8540A, B, C had been init-ially installed to isolate Reactor Coolant Pump Seal Leakoff Standpipe Level Switch LS406, LS407, and LS408 (Lov Level Alarms), as applicable. A change to the system deleted these level switches and the line was capped with the subject valves remaining in place.

During the preview of the Valve Lineup for RC-1, this fact was noted and the valve was marked "N/A".

There was confusion on the part of the operator as to what constituted " based on personal observation" as stated in Startup Manual Procedure SUM-B-8, and he believed that his assumption was valid.

Immediate Corrective Action Taken On August 19, 1980, Station Special Instruction No. 52 was issued by the Operations Supervisor which outlined actions to be taken when performing Valve Lineups. These requirements are as follows:

1) Any discrepancies must be noted and justified.
2) All valve positions must be physically verified (not inferred from Control Board indications).

If valve lineup is in error, i.e. valves no longer installed or new valves added to a system, note the changes on the Lineup Sheet.

4) Persons checking the lineup will initial beside the valves they personally check.

Shif t Supervisor will review the Lineup Sheets, resolve discrepancies, and sign "As Reviewed By (NOTE: On October 15, 1980, this was re-issued as Special Instruction No. 80-01.)
