~i Based on the NRC inspection July 21, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you 4
dated December 31, 1974.
4 As required by 10 CFR 50 Appendix B, Criterion V, implemented by FSAR Section 17.1.5 " Activities affecting quality shall be prescribed by docu-mented instructions, procedures, or drawings...and shall be accomplished in i
accordance with these instructions, procedures, or drawings." Start Up Manual Procedure SUM-B-8 " Conduct of Startup Testing", Section 3.4 states that initialing a step in a procedure indicates that based on personal observation or reports from assigned test personnel, the steps have been j
performed as written, conditions are as desired and results are as recorded.
Contrary to the above, on August 14, 1980 while performing a valve lineup as part of Preop Test Procedure RC-1 " Hot Functional Testing" valves 8353A, B, and C, 8357A, B and C were initialed as OPEN when in fact the valves do not exist. Valves 8540A, B, and C were initialed as not applicable "N/A" when in fact the valves exist and were closed.
This is an infraction.
