2 Event Date:

9-9-80 The Electronic Governor Reverse (EGR) actuator assembly connector plug was found cracked, shorting the wiring in both the male and female sections of the plug. This crack apparently was caused by personnel working in the area during the refueling / maintenance outage. The Electronic Governor Module (EGM) unit pickup cable plug, associated wiring, and the magnetic assembly were also damaged, which resulted in an inoperative EGM Control unit and Ramp Generator Signal Convertor (RGSC) unit. An open resistor in the dropping resistor as-sechly also resulted due to the damaged EGM unit cable and magnetic assembly.

11 addition, the valve actuator Servo valve was sticking due to infrequent use during the recent six month extended outage.

To correct these problems, the following components were repaired and/or re-placed and calibrated: EGR actuator and plug connector assembly, magnetic pickup and pickup cable plug, EGM control unit, RGSC unit, dropping resistor assembly, all associated wiring and the Servo valve. Following the repairs to the affected components, the HPCI turbine control valve V9 was successfully operated and tested thereby restoring HPCI to an operable condition.

To prevent possible reoccurrences a maintenance instruction is being developed l

to check for proper operation of vital HPCI system components following a r

major outage prior to reactor startup. All ECCS systems are routinely tested following maintenance and during normal operation to ensure that problems of this nature are defected.
