This refers to the special, onsite inspection conducted by the NRC Augmented
Inspection Team (AIT) during the period September 7-11, 1989.
The inspection
+
included a review of the events related to the overpressurization of the Unit 2
Containment Spray System and subsequent recovery.
At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed inspection report.

NRC considers this event to be a precursor of an ISLOCA.
The AIT concluded
that the overpressurization event was caused by a deficient test procedure and
an. operator error. The test procedure did not specify plant conditions limita-
,

tions' or warn the operators of the potential for overpressurization.
The
operators have not received specific training in the potential hazards of Inter
System LOCAs and failed to recognize the potential consequences of their actions.
The enclosed copy of the AIT report identifies the areas examined during
this inspection.
Within these areas, the inspection consisted of _ selective
. examinations of procedures and representative records, interviews with
!
personnel, and observation of activities in progress.

In. accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
