APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket Nos. 50-277 License Nos. DPR-44 and DPR-56 50-278 Based on the results of an NRC inspection.onducted on November 8 - 30,1980 it appears that certain of your activities w re not conducted in full compliance

-with conditions of your NRC Facility License Nos. DPR-44 and DPR-56 as indicated below.

Technical Specification 3.8.C.8 states in part:

... the off-gas radiation monitors shall be operable or operating whenever steam pressure is available to the air ejectors.

If these requirements are not satisfied, a normal orderly shutdcwn shall be initiated within one hour."

Contrary to the above, from about 6:30 a.m. November 23, 1980 to about 9:00 a.m.

November 24, 1980 the off-gas monitors were inoperable for periods in excess of one hour and no shutdown was initiated.

This Severity Level IV Violation is applicable to DPR-44, and is recurrent.

Technical Specification 6.11 " Radiation Protection Program," states, " Procedures for personnel radiation protection shall be prepared consistent with the require-ments of 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure."

Procedure No. HP0/CO-10A, " Conduct in Contrciled Areas Minimize Exposure,"

Revision 2, dated May 10, 1980, developed pursuant to the above requirements states in part, "Purcose: This procedure describes the rules which will govern the conduct of indivicuals assigned duties in controlled areas and outlines techniques for minimizing exposure...

Procedure:

Signs and radiation rope are posted at various areas to alert personnel to the radiation conditions which exist...

Padiation rope or tape is a barrier.

Do not cross such repe or tape...

It' area of interest has no step-off pad, contact Health Physics for information regarding entry."

Contrary to the above, on November 25, 1980, at approximately 2:30 p.m., three individuals disregarded special instructions on a sign and crossed through a rope barrier, at the south east side of the 165' elevation of the Unit 3 reactor building into a location which contained a high radiation area. The sign stated, "No entry, use west side."

This Severity Level IV Violation is applicable to DPR-56.
