SOUTH TEXAS PROJECT ELECTRIC GENERATING STATION RESPONSE TO NRC INSPECTION FINDINGS DOCKET NOS. S0-498/79-14 AND S0-499/79-14

Dear Mr. Seidle:

The following is Houston Lighting 5 Power Company's (HLSP) response to the items of noncompliance identified in IE Inspection Report Numbers S0-498/79-14 and 50-499/79-14, dated October 16, 1979.

A. Failure to Follow Procedures for Release of Stop Work Notice S-14 Corrective Action The Brown 5 Root Site QA Manager did verbally instruct a QC Inspector to sign a concrete pour card and allow concrete placement ME2-M3 to begin although Stop Work Notice S-14 had not been formally modified at that time. The basis for the decision was that Design Engineering had provided the Site with an authori:ed me-thod of testing the anchor bol: material to determine whether the material hardness was within the ASDI-A-36 or ASDI-A-193 range of Brinell hardness, thus providing adequate ma-terial identification. The tests were per-formed on the anchor bolts to be embedded in the specific pour, and evaluation of the test results verified they were of the proper ma-terial. The Stop Work was later modified to permi: continua:icn of the testing of anchor bolts and to provide for the release of sched-uled pours in the same manner. All project requirements for this placement have been met with the exception of releasing the Stop Nork before the documentation was complete.

B. Failure to Include Date of Entry of Identification of Person . Making Supolemental Entry Corrective Action The supplemental data noted on the reverse side of the referenced examination check for= was documented by the Lead Inspector responsible for the pours being made. The examination check form was presented to the individual, and he confirmed it to be his entry. He signed and dated the entry as a " late entry" on September 13, 1979.

Recurrence Control Interoffice memo, SQA-SS2, dated April 13, 1979, was issued to all QA/QC personnel stating that the identification of the individual making the entry and the date of entry are required for all corrections and additiens to docu=entation.
