Dear Mr. Smith:
In response to your letter dated August 13, 1980, and in accordance
with 10CFR2.201, the attached reply addresses the Notic! of '71olation which
was included as Appendix A of the referenced inspection repcct.
The noted
violation, identified as an infraction, consisted of the failure to complete
Form NRC-4 or an equivalent record.
We have reviewed the referenced inspection report for 10CFR2.790
information and none was identified.

Inspection No. 80-22
Letter Dated August 18, 1980
INFRACTION
Description of Infraction (80-22-01)
10 CFR 20.101, " Radiation Dose Standards for Individuals in Restricced
Areas", Section (a), limits the occupational whole body dose to 1.25 ram per
calendar quarter except as provided by 10 CFR 20.101(b), which allova an
occupational whole body dose up to 3 rems per calendar quarter provided that
certain specified conditions are met.
One of these conditicas requires that
the licensee has determined the individual's accumulated occupational dose
to the whole body on Form NRC-4 or on a clear and legible record containing
all of the information required on that form.
Contrary to the above, during the first calendar quarter of 1980, an
individual was permitted to exceed in excess of 1.25 rem (having received
1.41 rem) and the prescribed Form NRC-4, or equivalent, was not completed.
Discussion of Infraction
All Forms NRC-4, or equivalent (BVPS-Radeon Form 4.5), are reviewed by
the Radcon Foreman in charge of personnel mcnitoring.
The forms are reviewed
at the time individuals enter the station and complete required data on the
forms.
At this time, the Radcon Foreman assigns individual dose limits
depending on the degree of information available for form completion.
The
inspector noted the Radcon Form 4.5 was notated to limit the individual to
1.25 rem / quarter and, in addition, the individual's dose was controlled until
receipt of the indf ridual's prior exposure history. All required information
was available in tha individual's file but was not cranscribed onto Radeon
Form 4.5.
Corrective Action Taken
The omissions noted by the inspector were corrected immediately.
In
addition, personnel responsible for preparing the Radcon Form (s) 4.5 were
' instructed to promptly update Form 4.5 upon the receipt of the required
information.
Action to Prevent Recurrence
The Radeon Supervisor counciled both the Personnel Records Assistant
and his clerk as to the requirements for completing Radeon Form (s) 4.5.
In
addition, the Radcon Foreman in charge of Personnel Monitoring has been
instructed to personally review completed Radeon Forms 4.5 (Form NRC-4
l
equivalent) prior to permittini personnel to exceed occupational radiation
dose limits as set forth in 10 CFR 20.101(a) .
