O APPENDIX A NOTICE OF VIOLATION Duquesr,e Light Company Docket No. 50-334 Based on the results of an NRC inspection conducted on July 28-August 1,1980, it appears that one of your activities was not conducted in full compliance with NRC regulations and the conditions of your license as indicated below.
This is an infraction.
10 CFR 20.101, " Radiation Dose Standards for Individuals in Restricted Areas",
Section (a), limits the occupational whole body dose to 1.25 Rem per calendar quarter except as provided by 10 CFR 20.101(b), which allows an occupational whole body dose up to 3 Rems per calendar quarter provided that certain speci-fied conditions are met. One of these conditions requires that the licensee has determined the individual's accumulated occupational dose to the whole body on Form NRC-4, or on a clear and legible record, containing all of the information required on that form.
Contrary to the above, during the first calendar quarter of 1980, an indivi-dual was permitted to exceed in excess of 1.25 Rem (having received 1.41 Rem) and the prescribed Form NRC-4, or equivalent, was not completed.
