Excessive leakage from containment isolation valves AHV-lC and 1D contrary to Technical Specification 3.6.1.2.

Conditions Prior to Occurrence:

Mode 1 power operation (100%).

Descriptia7 of Occurrence:

At 1145 during performance of SP-179, Containment Leakage Types "B" and "C", it was discovered that containment isolation valves AHV-lC and 1D would not meet.the leakage requirements of the Procedure. The valves were closed and an investigation was initia-ted to determine the import of the situation. At 1340 it was de-termined that leakage exceeded allowable limits and created a condition requiring operation in accordance to Technical Speci-fication 3.0.3.

A power reduction toward Hot Standby was initia-ted. Operability was restored at 1351 after a repeat test per SP-179 and the power reduction was halted.

Designation of Apparent Cause:

The cause of this event is attributed to failure of the valves to seal at low ambient temperature during purge conditions.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

l Investigation of the event has included some limited testing to confirm the effects of low ambient temperature and development-of.a mechanism to prevent recurretice. Further' containment purge above Mode 5 will be restricted to testing of design changes implemented for problem resolution.

Failure Data:

1 This is the first occurrence. for excessive leakage from AHV-lC and ID, and the second event reported under this Specification.
