6.9.1.12.b Date of Occurrence: 11/20/80 Time of Occurrence: 0930 CST Identification and Description of Occurrence:

The auxiliary building gas treatment system (ALGTS) was declared inoperable due to a nitrogen supply line being run under the railroad door to the cask loading area preventing the door from completely closing.

It was unknown whether or not the ABGTS would maintain the required k" H 0 vacuum under this 3

condition.

During subsequent testing of the ABGTS at 2315 CST, vacuum relief damper, 0-FCo-30-149, failed in the open position. The ABGTS was again delcared inoperable sic.ce the k" H 0 vacuum could not be maintained.

3 Conditions Prior to Occurrence:

Mode 1 with reactor power at 49%.

Apparent Cause of Occurrence:

The nitrogen line having to be run under the railroad door was a recoenized con-dition.

Investigation discovered that this condition had not been tested during either preoperational or surveillance testing to verify the ABGTS would maintain the required k" H 0 vacuum with the line running under the railroad door.

3 The vacuum relief damper failed due to a faulty flow control operator.

Corrective Action

The nitrogen line was removed from under the railroad door at 0952 CST and the door closed completely. The ABGTS was later tested with the pipe reinstalled under the railroad door to establish if the required k" H,0 vacuum could be naintained under this condition. During the test the vacuum relief damper failed apen and was repaired within Technical Specification tine limits.

Af ter the damper was repaired the ABGTS was tested with the pipe run under the railroad door and the k

H O vacuum could not be maintained at the required 2

system flow rates specified in the surveillance requirements. The pipe was removed and the door closed. The A3GTS was retested and the k" H;0 vacuum was maintained.
