Containment isolation valves CAV-1, 3, 4, 5, and 126 are not capable of meeting the environmental qualifications of I. E.

Bulletin 79-01B due to submergence following a Loss of Coolant Accident (LOCA).

Conditions Prior to Occurrence:

Mode 1 power operations (46%).

Description of Occurrence:

At 1630 during evaluation of I. E. Bulletin 79-01B, it was discovered through conversations with the vendor and through engineering analysis, that ' containment isolation valves CAV-1, 3, 4, 5, and 126 would not meet the environmental qualification requirements due to submergence following a LOCA.

Designation of Apparent Cause:

The cause is attributed to potential electrical short due to flooding in the Reactor Containment following a LOCA.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public. Containment isolation valves CAV-2, 6, and 7 provide redundant isolation capability.

Corrective Action

Emergency Procedure EP-106 is being revised to instruct an operator to close and de-energize the subject valves in event of a LOCA.

In the interim, a Short Term Instruction has been issued which maintains these valves closed and de-energized ex-cept when needed for sampling. An engineering evaluation is in progress to resolve this potential problem.

Failure Data:

This is the first occurrence or event of this type and the first event reported under I. E. Bulletin 79-01B.
