- Facility: Crystal River Unit #3 Report Date: 18 December 1980 Occurrence Date: November 19, 1980 Identification of Occurrence:

Failure to have the Spray Additive System operable con-trary to Technical Specification 3.6.2.2.

Conditions Prior to Occurrence:

Mode 1 power operation (100%).

Description of Occurrence:

At 0915 on November 19, 1980 it was observed that level instrumentation BS-5-LI-l & LI-2 for sodium hydroxide tank BST-2 were erratic and displaying incorrect indication.

Maintenance was initiated and operability was restored at 1345 on November 19, 1980. At 0816 on 11/29/80 it was again observed that the level indication was erratic and incorrect. Maintenance was again initiated and operability was restored at 1430 on November 29, 1980. The volume of the tank remained within specification at all times.

Designation of Apparent Cause:

The cause of these events is attributed to water in the sensing line of the transmitter.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public. An investigation is being conducted to determine the cause of the water in the line, the extent of the i

problem, and corrective uction to prevent recurrence.

Corrective Action:

l The sensing lines were blown down and level returned to normal. Further corrective action will be implemented i

based on the results of the investigation in progress.

Failure Data:

i These are the first and second occurrences for BST-2, and this is the first event reported under this specification.
