In recent discussions with Messrs. J. T. Beard and D. Chaney of your staff, Duquesne Light Company has agreed to the following actions concerning resetting of the main feedwater and bypass regulating valves after a feed-water isolation signal. As a short term action, procedures will be reviewed and revised, as necessary, to require placing the controllers in manual and resetting the valves to the closed position. On-shift training will be provided to the operators for the procedure changes. The permanent corrective action will be to revise the control circuitry to prevent the valves from returning to their pre-trip position on resetting the ESF signal. This modification will be installed at the next scheduled refueling outage.
