August 20, 1979 MEMORANDUM FOR:
Robert M. Bernero FROM:
Peter Sicilia, Jr.
SUBJECT:
REVIEW 0F REGION I INCIDENT MESSAGE FORMS (R0-I, IMF)
I have reviewed the R0-I IMF's in detail for the first four days, March 28, 1979, to 11:50 PM March 31, 1979. The results of this effort are divided into three parts.
Part I - Four Day Chronology of Highlights Attached as Exhibit 1 is a chronology that reflects significant data elements documented by R0-I.
It highlights those events that are recorded and refer to the RCS status; plant parameters; significant leaks, releases, and expressed systems problems.
The chronology covers the period March 28 thru the 31st inclusive.
It should be considered a working document only and not complete in itself.
In many cases it reflects only significant parts of an entry as defined above.
Part II - Generic Summaries:
First 3 Days Based on the data available in the IMF from R0-I, I intend to prepare summary chronologies of the following areas:
1.
Evacuation 2.
High Pressure Injection System 3.
Names of People Involved or Mentioned 4.
Releases and Leaks 5.
Bubbles and Vacuums 6.
BWST 7.
Comunications 8.
Containment 9.
Depressurization/ Blow-Down/RHR-DHR System 10.
Fuel 11.
Pumps
- 12. Steaming and Condensors In all cases the summaries are written and will be typed this week. They will include all references made in the R0-I IMF during the first three days of the incident. These sumaries will be prepared and submitted by August 29, 1979.

Part III - Schedule of RCS - 1st Day
)
Attached as Exhibit 2 is a schedule of factual data as reported and documented in the R0-I IMF.
It reflects the RCS status and plant parameters throughout the first day, 3/28/79. This workpaper reflects the data received by R0-I and available to the NRC for analysis on that day. The individual reader can draw l
his own conclusions from this data.

C-56 8:45A Higgins iSeal return to makeup tank is causing excessive Ioff cas in makeup tank which in turn is directed j to waste gas tanks which are full.
Waste tanks being dumped to stack which is causing a~more ~ 3i considerable release rate.~~C/D and State are being notified by licensee.

C-57 9:15A Higgins Waste gas release problem. Waste gas flow path:
makeup tank, vent header, compressor, waste decay tanks 1 decay tank full, other one 1/2 full.
Rate of gas flow to vent header pressurized it, overloaded compressors, lifted relief - gases vented directly out stack.
