CAUSE DESCRIPTION AND CORRECTIVE ACTIONS (27)

The calibration of the rams used'in the surveillance test is the primary suspect contributing to the lift-off force anomaly.

Ra= calibration will be carefully ensured and verified using approved procedures. ? cst-surveillance calibration will be performed and lift-off data verified prior :o completion of the contract.

If the anomaly cannot be tied to the ram calibratics, appropriate action required by technical specifications will be taken.

To resolve possible discrepancies in dccumented tendon installation. ;isual inspections are planned to verify QC records on all tendons which will be tested

. in the spring of 1981. Such visual inspections could be expanded as a result of 4

these findings.
